Location: Medical Center - Fort WorthDepartment: Pharmacy-Dodson SpecialtyShift: First Shift (United States of America)Standard Weekly Hours: 40Summary: The Clinical Pharmacy Specialist will provide expert pharmaceutical care to patients with renal transplant or nephrological conditions, both in the ambulatory and inpatient settings. This role requires a deep understanding of complex transplant and renal pharmacotherapy and the ability to develop individualized treatment plans. Responsibilities include but are not limited to: Patient Care: Develop and implement comprehensive medication regimens for patients with renal transplant or nephrological diseases. Provide medication counseling and education to patients and their caregivers. Monitor patient response to therapy and adjust medication regimens as needed. Collaborate with healthcare providers to optimize patient outcomes. Clinical Services: Participate in transplant evaluation and post-transplant care. Manage medication-related adverse events and drug interactions. Provide consultative services to other healthcare professionals regarding renal pharmacotherapy. Research and Education: Stay current with the latest advancements in renal pharmacotherapy and clinical practice. Contribute to research projects related to renal transplantation and nephrology. Participate in educational programs for healthcare providers and patients. Qualifications:
