Patient Care Coordinator

Responsible for coordinating medical plans and treatments for patients. Discuss health concerns with clients, collaborate with physicians and other specialists to create comprehensive treatment plans, and educate patients about various health conditions and procedures.   

• Develop care plans for patients that address their individual health conditions and needs.
• Ensure that patients receive high-quality care and help coordinate their healthcare providers.
• Work with patients, families, and physicians to develop and implement treatment plans.
• Monitor the progress of patients and adjust their medical plans and treatments as needed.
• Support patients and their families and help them navigate the complex healthcare system.
• Coordinate transportation, schedule appointments, and assist patients with securing funding.
• Maintain a professional, empathic, and respectful demeanor toward patients at all times.
