As a Qualified Behavioral Health Provider (QBHP), you will help with achieving patient wellness and autonomy through advocacy, communication, education, identification of service resources and service facilitation.

Primary Responsibilities:

• Coordination of services both within and outside of Life Strategies Counseling ( i.e counseling, medication management, welfare, vocational services)
• Must achieve at least 60% Level of Care on individual and collateral visits
• Must conduct home visits at least once per month with client and guardian
• Provide transportation as needed for clients

Required Qualifications:

• Valid driver's license
• Must be able to travel independently

Preferred Qualifications:

• 1+ years of related experience and/or training
• Solid communication skills
• Ability to coordinate resources

The salary range for this role is $17.84 to $31.78 hourly. Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. UnitedHealth Group complies with all minimum wage laws as applicable.
