A Brief Overview
The discharge lounge liaison will provide outstanding customer service to all patients, guests, and staff. Responsible for identifying eligible patients, coordinating with nursing and care transitions, and staffing and monitoring the patient discharge lounge, including: transport from floor to lounge, patient check-in upon arrival to lounge, assigning patient to designated waiting area, and assisting patient to vehicle once transportation has arrived. Will follow all written regulations/procedures and correctly complete assigned tasks without supervision. Assist with other discharge areas/needs, as necessary.

What You Will Do



  • Politely and promptly meets and greets visitors, patients, and staff.
  • Daily rounding on patients with planned discharges for today and tomorrow to inquire about transport needs and introduce the Discharge Lounge.
  • Oriented to physical layout of hospital.
  • Rounding on patients to follow up on transport arrival.
  • Keeps environment clean & hazard free.
  • Provides safe and efficient transport to Main Entrance of Lerner Tower once ride arrives.
  • Ability to move patient safely when in wheelchair, as needed.
  • Appropriately triages patient and/ or family questions about discharge and ensure appropriate follow up/escalation to the appropriate staff.
  • Communicates any changes in patient condition or behavior appropriately.
  • Communicates with nursing and Care Transitions on patient transport needs.
  • Performs data entry on patients checked-in to the discharge lounge.
  • Receives dispatch information via Tele-Tracking System.

  • Performs other duties as assigned.
  • Complies with all policies and standards.
  • For specific duties and responsibilities, refer to documentation provided by the department during orientation.
  • Must abide by all requirements to safely and securely maintain Protected Health Information (PHI) for our patients. Annual training, the UH Code of Conduct and UH policies and procedures are in place to address appropriate use of PHI in the workplace.
