This position will be traveling to see patients primarily in the northern Seattle, Kenmore and Bothell area.

The MSW Social Worker is responsible for providing services to hospice patients, families, and caregivers, addressing a wide range of social work care coordination and psychosocial issues related to end-of-life needs including: adjustment to illness/prognosis, care system issues, community resource issues, insurance/financial issues, legal/ethical issues, and end-of-life planning. This position develops and implements a plan of care in collaboration with the Hospice Interdisciplinary Group (IDG) and is responsible for an assigned caseload shared with an RN Case Manager and a Hospice HA with the goal of promoting patient and family choice, ensuring continuity of care, and enhancing the quality of life of dying patients.

Required Qualifications:



  • Masters Degree from an accredited School of Social Work.
  • Washington Advanced Social Worker License upon hire. Or
  • Washington Advanced Social Worker Associate License upon hire. Or
  • Washington Clinical Independent Social Worker License upon hire. Or
  • Washington Clinical Independent Social Worker Associate License upon hire.


Preferred Qualifications:



  • 1 year of Post-Masters work experience
  • 1 year of prior internship experience in a Providence Health System setting
  • 1 year of prior experience in a healthcare setting
