Physical Therapist - Out Patient Rehab Services- Days

Department: Out Patient Physical Rehab Services

Location: Wellness Center on campus at Northwest Community Hospital

Hours: Must be able to work a minimum of 2 Saturdays per month.

Shift: 7.5 hour - 8.5 hour shifts. Shifts starts at 7 am

Part time/Full Time: Per Diem

What you will need:

• The completion of a degree in Physical Therapy from an A.P.T.A. accredited school, or the equivalent
• Current Illinois licensure or eligibility
• CPR certification through the American Heart Association

General Summary:

Reporting to the Rehab Services Manager or Rehabilitation Supervisor with accountability to the Director and Team, performs evaluations, treatment planning, goal setting and develops and implements individual Physical Therapy programs designed to improve and maximize patient mobility and independent functioning within their physical limitations.
