Job Summary

The SUD Case Manager position is part of the Behavioral Health Services team at the 1930 Market Street Service Center and is responsible for providing case management services to individuals and groups who have substance use disorders, potential co-occurring mental health disorders and/or are living with HIV/AIDS. This position is part of a larger multidisciplinary behavioral health services team that includes psychiatrists, nurses, social workers, therapists, substance use counselors, case managers and psychologists.

This position provides case management services to individuals living with co-occurring diagnoses of substance use issues, mental health issues and medical issues, within a Harm Reduction philosophy. Motivational Interviewing, among other interventions, is utilized to help clients explore their relationship to their drug(s) of choice, address their ambivalent feelings about making changes to their use and creating client-centered, realistic and meaningful treatment plan goals for change. This position works under the clinical supervision of a certified substance use counselor and in conjunction with other teammates providing outpatient-level substance use treatment services. This position will involve providing case management services consisting mainly of field work, outreach and navigation of clients to community services, assessing clients' service needs and assisting clients with appropriate referrals (e.g., substance use treatment, housing, vocational and other mental health services) to reduce barriers to treatment. Additionally, the SUD Case Manager will assist clients in creating and meeting treatment plan goals and reduce barriers to care.

The final salary and offer components are subject to additional approvals based on UC policy.

Your placement within the salary range is dependent on a number of factors including your work experience and internal equity within this position classification at UCSF. For positions that are represented by a labor union, placement within the salary range will be guided by the rules in the collective bargaining agreement.

The salary range for this position is $29.79 - $38.74 (Hourly Rate).

Department Description

The Alliance Health Project (AHP) is a community-based LGBTQ+ and HIV-focused clinic located at 1930 Market Street in San Francisco. We provide psychotherapy, substance-use counseling, case management, support groups, psychiatry, sexual health services, clinical traineeship, and provider education and training. Our multi-disciplinary staff are a client-centered, collaborative, Harm Reduction-oriented team. We have continued to grow as a community-based clinic by anticipating and addressing the changing needs of our LGBTQ+ and HIV community members for more than 40 years. Our process of continuous improvement of service to our community includes a strong focus on cultural humility and anti-racist service provision.

Required Qualifications

* Possession of a bachelor's degree in social work from a program approved by the Council on Social Work Education, or a bachelor's degree in a related field, and one year of experience; or an equivalent combination of education and experience; and knowledge and abilities essential to the successful performance of the duties assigned to the position.
* One year of experience in providing case management services in professional/clinical setting
* At least 6 months of experience working within a multidisciplinary team.

Preferred Qualifications

* Fluent in Spanish, with written and verbal proficiency and past professional experience utilizing Spanish in clinical work.
* 1 year of professional experience providing case management services to Black/African American and Persons of Color (BIPOC) clients
* Demonstrated commitment to Antiracism, Cultural Humility and Social Justice practices in clinical work
* Experience working within Harm Reduction treatment models, utilizing Motivational Interviewing interventions
* Certified, and/or registered, as Certified Addictions Treatment Counselor (CATC) or other addiction-related certification/registration through the California Association of DUI Treatment Programs (CADTP) or California Consortium of Addiction Programs and Professionals (CCAPP)
* Familiar with Avatar, ARIES, Epic and CBHS SF Mental Health and Drug Medi-Cal systems of care,
* Understanding of psychosocial issues facing the LGBTQ and HIV-affected communities.
* Experience working within Harm Reduction treatment models, utilizing Motivational Interviewing interventions
* Familiar with community-based case management services, linkage and/or intensive case management services.
* Knowledge of local SF based community resources for linkages to services
* Ability to work independently in non-office based and community settings in case management work.
* Excellent communication and interpersonal skills.
* Demonstrated excellence in reliability and attendance.
