Our Active Consulting program in Arkansas is currently recruiting for a Service Coordinator to provide direct services and support to agency providers, caregivers and family members in serving persons with Intellectual and Developmental Disabilities (IDD) and/or mental health diagnoses in crisis or at risk of inpatient placement, involvement with law enforcement, potential harm to themselves or others, or loss of residential/home placement due to severe behavioral and/or psychiatric related issues. GENERAL RESPONSIBILITIES: The Service Coordinator will also provide education and training to provider agency personnel, caregivers and family members in support of the individual's treatment and care. They must be willing to provide clinical and operational support to identified staff under the direction and supervision of an independently licensed professional. ESSENTIAL FUNCTIONS AND RESPONSIBILITIES:
- Provide in person and remote training to provider members and staff, including Direct Support Professionals (DSPs), caregivers, family members and other community providers.
- Complete all documentation of services provided per policy.
- Provide clinical recommendations and training to participating providers.
- Provide direct monitoring, assessment, intervention, and training on behavioral support plans, safety plans, mental health treatment plans, proactive strategies, precipitants, staff interventions, replacement behaviors under the supervision of an independently licensed mental health professional or BCBA.
- Maintain system for collecting, recording, maintaining and reporting program specific information including key performance indicators.
- Assist in completion of mental health or functional behavior assessments under the supervision of an independently licensed mental health professional or BCBA.
- Develop and implement mental health treatment, behavior support, and safety plans in areas of competency under the supervision of an independently licensed mental health professional or BCBA.
- Establish and maintain effective communication strategies with Team members
- Assist with modifying mental health treatment and behavior support plans as necessary.
- Attend all agency, departmental, and client specific meetings as directed.
- Assist in transport of individuals according to Benchmark Policy.
- Review incident reports and incidents of physical intervention in coordination with Program Director and Compliance.
- Assist Active Consulting Team members in case management functions and discharge planning.
- Must report any suspected abuse, neglect or exploitation to Program Director and appropriate state authorities according to state rules and regulations.
- Comply with all organizational safety standards to assure the health and safety of all individuals.
- Must be willing to flex schedule accordingly, i.e., on-call, work evenings and weekends, to provide services, supports, and treatment to individuals as required.
QUALIFICATIONS: Minimum:
- In a graduate program pursuing a master's degree in a field related to applied behavior analysis, social work, or clinical counseling
- At least two years' experience with behavior supports evaluation and services for people with intellectual and developmental disabilities or complex behavioral needs
- Trained in crisis de-escalation methodology
- Valid CPR and First Aid Certification
- Valid Driver's License and auto insurance
- Must be computer literate
- Must be experienced and competent in profession and maintain any applicable training, license and/or certification.
Preferred:
- Minimum of four years' experience working with individuals with intellectual and/or developmental disabilities and/or mental health diagnoses who are in crisis
- Working knowledge of the Arkansas DHS service system
