Elwyn is seeking an Adult Day Instructor to provide direct support to people with physical, mental, and intellectual disabilities within day programs, ensuring the health and wellbeing of the individuals in which they serve as well as promoting personal independence.

ESSENTIAL FUNCTIONS:



  • Supervises and is accountable for all individuals assigned to his/her group/program area
  • Supervises, instructs, supports and provides guidance to assigned persons served in accordance with the individual's support plan, and working daily toward the designated outcomes, including but not limited to:


    • ethnic and cultural support
    • recreational/socialization activities
    • physical fitness
    • healthy living
    • self advocacy
    • independence and choice
    • cooking
    • housekeeping
    • money management
    • social awareness
    • interpersonal relationships
    • personal hygiene


  • Coordinates with program supervisor to assist with daily attendance and accounts for the whereabouts of assigned individuals throughout the program day
  • Ensures all individuals in his/her group are actively engaged in an appropriate program activity consistent with the program design, mission, and activity schedule
  • Provides support to individuals while engaged in community activities which keep them safe and engaged
  • Completes paperwork and maintains all assigned documentation as required and as determined by program management for the program site
  • Submits daily progress notes in a timely fashion and by required deadlines
  • Promotes a positive learning environment
  • With assistance from the Program Supervisor, collaborates in the development of training programs
  • Completes related work as assigned in a timely fashion
  • Attends in-service training sessions as needed and required by regulations and standards
  • Provides personal care to individuals as needed
  • Participates in new program initiatives that support mission and purpose, providing that new initiatives do not increase instructor's job duties and remaining operations stay in ratio compliance
  • Responds accordingly to emergency situations based on Elwyn policies and procedures
  • Attends Individual Service Plans (ISP) meetings, team meetings and staff meetings as scheduled or requested by the program supervisor
  • Documents (e.g. tracking sheets) and tracks any behaviors covered under any Behavior Management Plans as developed by the team, and properly implements and follows behavior management techniques in accordance with the plan
  • Assists with planning of trips, special events, activities, etc., and participates in such trips, special events, activities, etc.
  • Serves as an appropriate role model for individuals as well as other employees
  • Ensures that individual rights are protected and reports any violations of individual rights immediately to the Program Supervisor, Program Manager or Department Director or QI department
  • Assists in the resolution of problematic situations that may occur for individuals supported; notifies program management of any unusual situations, emergency situations or any problems unable to be resolved
  • Ensures that individuals have all prescribed adaptive devices available to them (i.e., glasses, hearing aids, etc.) and maintain as needed
  • Assists in all activities of daily living as needed for the individuals
  • Maintains a clean and safe work environment for all individuals
  • Follows housekeeping standards and will report any health or safety concerns to the Program Supervisor or designee and ensures there is compliance with the New Jersey State Department of Human Services, Division of Developmental Disabilities (DDD) Supported Employment/Day Program Manual, applicable state, local and federal regulations, etc.
  • Accepts reassignment to support individuals in other locations or areas when and as needed
  • Maintains current state-mandated training and certifications
  • Follows all current policies and procedures of Elwyn New Jersey
  • Cooperates with Elwyn New Jersey and the State of New Jersey Department of Human Services, or any other authorized investigating body during an inspection, inquiry, or investigation
  • Performs cardiopulmonary resuscitation (CPR), safe crisis management, and medication administration using agency-trained protocols
  • Performs other duties as assigned


QUALIFICATIONS, EDUCATION AND EXPERIENCE:



  • High School Diploma or graduate equivalency diploma (GED)
  • Minimum age requirement of 18 years old
  • Experience in a related field or with individuals with developmental disabilities preferred
  • Must successfully complete and obtain/maintain all certifications as required by Elwyn New Jersey or regulatory agencies
  • Non-profit human service experience highly preferred
  • Demonstrated ability to work effectively as part of a team
  • Ability to work in a fast-paced environment and manage and prioritize multiple, often competing, priorities
  • Demonstrated strong attention to detail
  • Demonstrated strong time management and organizational skills
  • Must be able to perform CPR and safe care using agency-trained protocols
    Demonstrated basic experience with Microsoft Office applications, including Word, Excel, and Outlook preferred
  • Experience using/knowledge of Electronic Health Record (EHR) / Electronic Medical Record (EMR) preferred
  • Must submit to a criminal background check and drug test and be determined to be not disqualified by the New Jersey Department of Human Services prior to working with individuals served
  • Must agree to have their name checked against the New Jersey Central Registry of Offenders Against Individuals with Developmental Disabilities and NJ Child Abuse Registry (CARI)
  • Must have current, valid driver's license in state of residence, three (3) years driving experience in the United States, and acceptable driving record
