Summary: Responsibilities: 2. Monitors for potential discharges and helps coordinate a timely discharge. 3. Communicate admissions to the accepting teams. Communicate admissions to Bed Assignment & coordinate admitting times with the House Supervisors. Serve as a point of contact for any stakeholder inquiring about the admission of a patient. Troubleshoots and manages conflict. 4. Takes action to follow up on safety concerns of patients, families, providers or staff. 5. Leads and participates in quality improvement initiatives that focus on identified patient flow and satisfaction deficits.
