Responsible for outreaching and engaging newly authorized enhanced care management clients with the goal of enrollment to assign to a lead care manager.SPECIFIC RESPONSIBILITIES:

This position is contingent upon the availability of funding, which is expected to last for 3-6 months. The role may be extended based on the availability of additional funding and project needs.
