Outreach & Engagement Coordinator

With over 23 programs, we offer multiple ways to aid seniors maintain their health, well-being, independence and participation in the community, fulfilling our mission.

Responsible for outreaching and engaging newly authorized enhanced care management clients with the goal of enrollment to assign to a lead care manager.

SPECIFIC RESPONSIBILITIES:

  • Initiate telephonic contact with newly eligible enhanced care management potential clients after authorization received by the health plan.
  • Complete face to face visits to enroll clients if they agree to home visit.
  • Engage in culturally and linguistically appropriate client services as part of community served by program.
  • Responsible for providing education to potential clients and community organizations about program and services provided.
  • Obtain consent to receive services and enroll new clients.
  • Maintain contact with new enrollees until engagement with lead care manager.
  • Ensure accurate and timely documentation in program's electronic health record of all outreach activity.
  • Maintain open communication with supervisor to track referral and enrollment statuses across programs.
  • All other reasonably related jobs as assigned.

BACKGROUND AND EXPERIENCE:

Minimum Requirement:

  • One year of experience working with the disabled-adult or underserved population.
  • Lived experience in community served and/or with client experience.
  • Detail-oriented with successful organizational skills.
  • Strong ability to clearly, effectively communicate.
  • Bilingual English/Spanish required.

Preferred Requirements:

  • More than one year of experience working in home health care coordination and community-based organizations working with at-risk population
  • Experience working with health plan organization and/or community case management programs.

This position is contingent upon the availability of funding, which is expected to last for 3-6 months. The role may be extended based on the availability of additional funding and project needs.
