Responsibilities:
- Provides quality Mental Health services to consumers with a mental health disorder
- Completes progress notes and other required documentation in the electronic consumer files within the required timeframe.
- Provides services to or on behalf of the person served
- Services may be provided in the community or in one of the program locations they may include Crisis Intervention, assisting with applying for benefits, activities of daily living, psychosocial support, service coordination, linkage, transportation, accessing community resources and/or housing/housing retention.
- Establishes and maintains working relationships with community resources, coordinates consumer treatment with other providers, and initiates referrals to medical providers and other community resources when appropriate.
- Performs other duties as assigned
- Provides training and guidance to new staff in addition to current staff that need more direct assistance.
- Ensures that Day Support services are delivered in a timely and efficient manner.
- Provide oversight and guidance on Day Support services to ensure that client receive high quality treatment in a supportive and meaningful way.
- Train and support staff to adhere to all Integration expectations and standards including, but not limited to, Whole Person Care documentation
- Lead the team staffing's with Case Managers when meeting with other departments in the agency.
- Establishes and maintains working relationships with community resources
- Communicates with internal and external interdisciplinary team members to facilitate good clinical management of individuals with serious mental illness and coordinates treatment with other providers
- Completes necessary paperwork as required, meeting established timelines and making authorized entries in the medical record
- Fosters teamwork and cooperation between team members
- Identifies and prioritizes issues and concerns that need resolution. Includes others in the decision-making process when appropriate to make independent, effective decisions using available information.
- Maintains confidentiality in accordance with HIPAA regulations and educates individuals on matters requiring informed consent.
- Participates in ongoing Continuous Quality Improvement to assure the services provided demonstrate best practice standards and meet all contractual requirements
Qualifications
Job Requirements: The following requirements are those that are normally required for performance of this position. Physical Requirements: This position normally requires that physical demands of standing, walking, bending, lifting or performing other work requiring low physical exertion, talking and hearing on a regular basis to perform the job requirements. These physical demands are required up to 80% of the time. Position also requires ability to drive with adequate vision and skill. Non-Physical Requirements: Education
- Bachelors degree in Behavioral Sciences or Social Work, preferred
- Associated degree in Behavioral Sciences or Social Work, required
Experience
- 3 years experience in direct Clinical treatment services - required
- 1-year experience in treatment with group treatment and day support - required
Special Skills
- Strong family and individual Clinical Case Management skills
- Possess and utilize effective verbal and written communication skills
- Ability to be self-motivated, work independently and as a team member
- Good understanding of CSCW P&P
- Bilingual (English/Spanish) preferred but not required
Licensure, Registration, Certification
- Valid Washington State drivers license and minimum required liability insurance for WA State
- Must be deemed insurable as determined by Catholic Charities liability insurance provider
- Agency Affiliated Counselor Registration
