Overview: LPN - Intake Specialist, Behavioral Health Urgent Care

Heritage Behavioral Health Center is seeking a full-time LPN position in our Urgent Care HUB. This unique role combines clinical nursing responsibilities with intake and triage functions, supporting individuals with serious mental illness and chronic health conditions.

As an LPN, this position will provide both primary care and psychiatric support, helping clients manage their health through direct services, medication support, and health education. In addition to clinical duties, the LPN will be cross-trained as an Intake Specialist, serving as a key point of entry for individuals seeking care. In this role, the nurse will perform behavioral health triage, conduct rapid assessments, and help connect individuals to appropriate services and resources within Heritage and the broader community.

This is a full-time position requiring a commitment of 40 hours per week, Monday through Friday, from 8:00 AM to 5:00 PM.

Core Responsibilities:

  • Accurately obtain and document vital signs, including blood pressure and other key health indicators.
  • Administer prescribed medications as ordered, and promptly report any adverse reactions to the appropriate medical staff.
  • Monitor patients' health status and ensure timely, thorough, and accurate charting in medical records.
  • Communicate changes in patient condition clearly to providers.
  • Maintain readiness by ensuring all supplies, medications, and equipment are properly stocked, organized, and in working order.
  • Coordinate with the pharmacy to ensure timely and accurate medication refills and adjustments.
  • Facilitate referrals and linkage to appropriate healthcare services for patients requiring ongoing medical care or support.
  • Assist with triage, assessment, and coordination of care by collaborating with healthcare professionals to address a variety of urgent healthcare concerns.
  • Interact with patients and families in a compassionate, respectful, and professional manner to support a positive care experience.
  • Provide a warm, welcoming first point of contact for individuals seeking behavioral health services and support.
  • Conduct behavioral health triage to assess immediate needs and determine appropriate levels of care and intervention.
  • Complete comprehensive needs assessments to identify medical, behavioral health, and social service needs, including mental health, substance use, housing, food security, transportation, and employment.
  • Administer assessments and screening tools such as suicide risk assessments, IM-CAT, PHQ-9, GAD-7, and screening for social determinants of health.
  • Coordinate and facilitate access to care by scheduling intake appointments and assisting with transportation arrangements when necessary.
  • Provide advocacy and feedback regarding client and program needs to enhance service delivery.
  • Attend required trainings and team meetings to stay current with best practices in behavioral health triage and care coordination.
  • Perform other duties as assigned or required by the supervisor.

Knowledge, Skills and Abilities:

  • Ability to engage individuals and decrease barriers to accessing treatment
  • Ability to solve problems and multi-task and coordinate a high level of activities.
  • Considerable interpersonal as well as oral and written communication skills
  • Ability to work in a team-based environment
  • Strong customer service skills
  • Ability to respond effectively in an emergency situation for patient care, possess strong listening and communication skills, and be attentive to detail.

Education and Experience:

  • Active undisciplined State of Illinois Licensed Practical Nurse (LPN) licensure.
  • Excellent organizational skills and attention to detail.
  • Ability to work independently and as part of a multidisciplinary team.
  • Compassionate and empathetic demeanor with a commitment to patient-centered care.

A valid driver's license, reliable means of transportation, and proof of automobile insurance are required.

Salary Range: $26.00 - $35.00 per hour - dependent on education and experience. $2,500 sign on bonus included!
