Position Summary:


The RN Intake Coordinator interprets and applies established admission criteria for Carle Home Health Services product lines to prospective patient referrals. Ensures the timely and efficient transmission of referrals to appropriate operational area and ensures behaviors that promote customer satisfaction. Hours may vary depending upon census and program need.

EDUCATIONAL REQUIREMENTS
College Diploma Nursing

CERTIFICATION & LICENSURE REQUIREMENTS
Registered Nurse (RN) Illinois and Basic Life Support (BLS). Attend and satisfactorily complete all required continuing education regarding the care of acute stroke patients.

EXPERIENCE REQUIREMENTS
One (1) year Healthcare/Medical

ADDITIONAL REQUIREMENTS

  • Attend and satisfactorily complete all required continuing education regarding the care of acute stroke patients.
  • Continuing education requirements for this position will include content that is assigned by the organization to comply with DNV Hip and Knee Replacement Certification requirements. This content will only apply to those directly assigned to the total joint replacement practices. This requirement is subject to change pending updates to DNV Hip and Knee Replacement Certification standards.


SKILLS AND KNOWLEDGE
Knowledge of home health, hospice, home infusion, case management, Medicare, Medicaid, federal and state accreditation standards, and experience in dealing with third party payers. Strong computer skills and Medical terminology.


Essential Functions:

  • Collects demographic and clinical data to support admission into home health programs and completes entry of information, orders, etc. in computer system
  • Communicates and coordinates with other team members, sharing information to enable the most complete possible assessment of needs and requirements for both patient and organization.
  • Completes benchmarking data for all programs as assigned.
  • Develops fundamental knowledge of diagnosis coding and makes accurate recommendations to field staff for new admissions.
  • Identifies patient situations and conditions that may limit or preclude successful admission to home health program; works with referral sources and dept. mgmt to facilitate and resolve.
  • Performs basic scheduling functions, if requested, during periods of staffing shortages.
  • Prioritizes work and Coordinates with scheduler to arrange timely admission into Home Health programs.
  • Works with referral source to establish medical necessity, skilled need, diagnosis, physician to order/follow patient; viable funding sources, etc. and documents in EMR.
  • Ability to work comfortably between EPIC and Remote Client to ensure timely and accurate documentation with each new referral.
  • Ability to study information provided in the new referral to ensure that patient meets guidelines mandated by Medicare for skilled need and home bound status.
  • Monitor insurance coverage to determine what, if any co-pays patient will need to be informed of and to ensure that CHC is in-network with patient's insurance coverage.
  • Verify that patient has MD willing to sign home care orders and to ensure patient is current with this MD to ensure compliance with F2F completion.
  • Complete patient referral thoroughly and accurately following established home care referral templates
  • Monitors daily status of all home care patients that have been transferred to inpatient status, fax transfer forms to inpatient facility and update referral with new resumption of care orders once patient is discharged.
