Manager of Clinical Integration

Position Summary:


The Clinical Integration Manager is a highly strategic role within Carle Health Physician Partners (CHPP), designed to provide value to the physician and offices through expertise, consultation, and process management. CHPP is a physician-driven clinically integrated network dedicated to delivering high value, evidence-based coordinated care to drive clinical excellence and improve health outcomes. The Clinical Integration Manager supports clinical integration efforts within the CHPP network, including strategy development around quality and utilization measures, ACO initiatives, care coordination and patient experience.

The manager is responsible for meetings regularly with CHPP physicians and practices. The purpose of these meetings is to establish and maintain strong physician engagement and assist practices in maximizing performance in the clinical integration program. At the practice visits, the manager delivers performance reports to physicians, identifying care gaps and improvement opportunities. The incumbent must possess excellent interpersonal skills to effectively communicate with physicians about performance improvement opportunities. The manager will develop strong working and collaborative relationships within the Carle Health system to support the network's goals.

This position is a highly visible role representing CHPP within the physician community. The manager works closely with CHPP and senior leadership to ensure membership growth and expansion into new markets by executing the vision of CHPP aligned with Carle's organizational strategy. The Clinical Integration manager will be responsible for CHPP marketing and communications as well as highlighting new physicians to promote network integrity.

EXPERIENCE REQUIREMENTS
  • Three (3) or more years in related field

SKILLS AND KNOWLEDGE
  • Expertise in healthcare operations management, quality, performance improvement, payer contracting, and business development.
  • Expertise in population health and managing patient databases to improve quality and lower costs.
  • Continual learning skills through examination and established research, best practice and current healthcare trends.
  • Adaptability and acceptance of rapidly evolving work environment.


Essential Functions:

  • Support managed care issues, credentialing, data analytics, performance reporting, and quality improvement.
  • Relationship Development & Management
  • Manages assigned practices to engage physicians, meeting monthly with PCP offices, quarterly with specialists.
  • Share reports, patient lists and dashboards with the physician
  • Quickly identify performance issues
  • Articulate the data in a way that expedites the physician's understanding of the data elements.
  • Integrate use of claims data and analytics
  • Identify process improvement opportunities for better population health management
  • Enhance coordination of care, offering solutions to providers as needed.
  • Routinely shares feedback to maximize physician engagement and CIN development.
  • Actively participates in activities surrounding the implementation, measurement and improvement of Carle Health Physician Partners.
  • Support board of directors and operating committees by preparing meeting agendas, minutes, presentations and reports.
  • Supports network growth and performance by facilitating physician achievement of value-based care goals.
  • Coordinates marketing and communications for operations including biweekly newsletter, creating practice agendas and informational resources for dissemination.
  • Effectively communicates CHPP value offerings to match physician and practice needs.
  • Facilitate trainings on population health tools, website and analytics software as needed.
  • Responsible for correspondence and follow-up with physicians and practices.
