Family Health Centers of San Diego is looking for an Associate Director, Social Services. Under the supervision and guidance of the Director of Social Services, the Associate Director provides operational management, administrative and clinical oversight of the department's Care Management and Social Services delivery.

Responsibilities:

  • Build positive relationships, inspire others to perform at a higher level, prepare career development plans and retain an accountable workforce.
  • Coach and counsel staff and administer appropriate disciplinary action in a timely manner.
  • Ensure integration of social workers in the medical clinic setting to support patients, their families, and the interdisciplinary care team.
  • Responsible for the overall administration and the day-to-day functioning of assigned Social Services Teams.
  • Oversee staffing and regulatory support for continued growth of the social services programs and integration, and care management activities.
  • Integrate social services operations in supporting patients, their families, and the interdisciplinary care team.
  • In conjunction with Director, participate in strategic planning and program budgeting.
  • Work effectively with IT's Business Intelligence team to develop strong performance metrics and measures of impact adjusting strategies based on outcome data.
  • Responsible for the overall administration and the day-to-day functioning of assigned Social Services Teams.
  • Support organization-wide expansion of screening methods for SDOH, SOGI, PHQs, and other assessments that lead to the connection of care management services and improved patient outcomes.
  • Ensure appropriate delivery and standardization of FHCSD episodic and longitudinal care management services.
  • Responsible for developing, standardizing, and maintaining Care Management staff competencies and job skill assessments.
  • Foster and sustain FHCSD's community hospital relations to maximize referrals and promote positive patient care transitions.
  • Support hospital-based social services to increase patient transitions to primary care and mental health services at FHCSD providing clinical appropriateness.
  • Provide individual and group Clinical Supervision for Associate Clinical Social Workers seeking hours towards licensure.
  • Participate in continuous improvement activities and ensure appropriate Lean tools (i.e., 5S, A3) are deployed and results are evaluated and/or measured.
  • Perform other duties as assigned.

Requirements:

  • Current California State clinical licensure.
  • Master's Degree in social work, psychology, counseling or closely related field required
  • Must meet driving requirements.
  • 5 years of experience in a community based organization, which must include at least 3 years progressively responsible experience in the health care/behavioral health industry.
  • 1 - 2 years of experience in providing and/or supervising social services preferred.
  • Ability to maintain good interpersonal interactions with clients and co-workers as a member of a multidisciplinary community health team with a diverse multi-cultural population.
  • Strong analytical and problem-solving skills to include critical thinking skills.
  • Strong verbal and written communication skills with the ability to positively interact with a broad and diverse range of cultures, socio-economic groups, and personnel.
  • Knowledge of supervisory practice and techniques to include performance management and employee development.
  • Bilingual in Spanish/English strongly preferred.
  • Demonstrated ability to be culturally sensitive and respect diversity.
  • Demonstrated organizational skills with attention to detail that include time management skills, with a strong motivation in meeting deadlines and achieving established goals.
  • Effective training and coaching skills; ability to drive results and achieve objectives through others.
  • Good written and verbal communication skills, including ability to communicate effectively with patients and family members who speak English as a second language.
  • Knowledge of budgets, including budget preparation and oversight.
  • Knowledge of functions, services and positions of the program(s) under area of control.
  • Demonstrated skills in computer applications and software, including MS Office, data collection, and patient tracking.

In the spirit of pay transparency, we are excited to share the base range for this position, exclusive of fringe benefits or potential bonuses.

If you are hired at Family Health Centers of San Diego, your final base salary compensation will be determined based on factors such as geographic location, jurisdictional requirements, skills, education, and/or experience. In addition to these factors - we believe in the importance of pay equity and consider internal equity of our current team members as a part of any final offer. Please keep in mind that the range mentioned above is what we reasonably expect to pay for the role. Hiring at the maximum of the range would not be typical in order to allow for future and continued salary growth.
