Qualifications



  • Bachelor's degree in human services or related field.
  • Experience in providing services to individuals or families that have experienced homelessness, HIV/AIDS, behavioral health challenges, chemical dependency, and/or in supportive housing.
  • Understanding of the delivery of social services and eligible benefits in relation to the above fields of knowledge.
  • Ability to accept supervision and work with a team of co-workers.
  • Ability to work well with clients of diverse cultural and socioeconomic groups; respect confidentiality; able to work within ethical and legal parameters of social services.
  • Willingness to work in an environment with a flexible work schedule and in a team environment.
  • Ability to use Salvation Army approved word processing, spreadsheet, database management, and graphics/desktop publishing software packages.
  • Valid Driver's License. (Must meet the guidelines provided by the State of residence and the automobile insurance provider as pertaining to corps' passenger vans/buses.)
  • Must pass The Salvation Army background checks and meet the MVR driver
  • Must complete The Salvation Army Caseworker Certification Program within twelve (12) months of hire.
  • Willingness to support the philosophy, goals, objectives, and mission of The Salvation Army.
  • It must be understood by the incumbent that the provisions of Safe From Harm will apply upon commencement of employment and also understood that the required Safe From Harm training be satisfactorily completed within the first thirty (30) days of employment.


Summary of Position

The Case Manager is responsible for establishing and maintaining working relationships with the residents of HOPE Harbor and Harvest Hills by implementing a case plan, establishing goals and evaluation of services provided, and by providing direct services to residents in a professional manner.

40 hours/week, $23.00/hour

Essential Duties and Responsibilities

Case Manager or the Program Director II.



  • Maintain positive work atmosphere by behaving and communicating in a manner that fosters good relationships with co-workers and supervisors. This would include but not be limited to such actions: resolution of conflicts in a professional manner; courteous treatment of staff, visitors and clients; respect of others' property and person, professional and appropriate communication to and about co-workers, supervisors, and subordinates.
  • Support the Mission of The Salvation Army by treating every client, donor, and colleague with respect and dignity, and without discrimination of any kind.
  • Adhere to the guiding principles of The Salvation Army's social services ministries as outlined in Faith In Action with an emphasis on intentional outreach in the area of holistic care to those we serve.
  • Engage all social service clients (including and not limited to, POH, EH families/children, etc.) in a way that brings them to the Corps for programming, events, and activities.
  • Be knowledgeable of all Salvation Army Social Service and Corps programs.
  • Provide direct case management to residents at HOPE Harbor and Harvest Hills
  • Complete assessment, and implement and establish a case plan for each resident, maintain and document appropriate clients records and maintain statistical information.
  • Provide crisis intervention assistance (mental health, chemical health, physical health)when needed.
  • Assist individuals with all aspects of direct services, including move-in and move-out procedures, case management services, advocacy and referrals.
  • Plan and facilitate resident educational and social activities on a monthly basis.
  • Work independently and work between multiple locations.
  • Transport residents as needed.
  • Collect documentation and verify status of qualifying diagnosis, chronically homeless, long-term homeless, and seriously mentally ill residents.
  • Accurately enter data into multiple systems (HMIS, MHIS, etc).
  • Maintain required statistics as directed by the funding providers and the Program Director.
  • Participate in office and department meetings, communities and other networking
  • Participate in community meetings and networking.
  • Facilitate educational/support groups and the statistics as needed.
  • Assist in planning and development of seasonal activities as requested.
  • Participate in meetings with office staff, building staff and the Property Management Company.
  • Attend educational opportunities when appropriate.
  • Promote Pathway of Hope throughout the community and make Pathway of Hope referrals as appropriate.
  • In absence the position will covered by the Clinical Lead Case Manager or another staff as assigned
  • Other duties that are specific to the office location and as assigned.


Working Conditions

There are normally no hazardous or significantly unpleasant physical working conditions, with the exception of the occasional emotional distress or turmoil (due to varying situations) with and between staff and/or clients. The duties and responsibilities of this position cannot be performed from home
