Description
Position Summary:
The Case Manager collaborates with WestCare staff and external bodies to provide assessment, counseling, re-integration, and/or rehabilitation services to program participants. Essential Job Functions include those listed below.
Essential Job Functions:
* Provides case management services and assistance to program participants struggling with substance abuse, and those in recovery;
* Assesses the needs, conditions, and desires of assigned program participants;
* Educates and refers program participants to community agencies and businesses that appropriately address their needs for food, shelter, clothing, employment, transportation, etc.
* Assists participants with establishing or re-establishing community support systems;
* Acts as advocate on behalf of program participants to improve access to vital services;
* Participates in clinical staffing decisions regarding the participant's current strengths and resources, and their need for additional resources;
* Completes documentation of services provided in a timely manner;
* Provides input into the patient's treatment needs and disposition;
* Conducts assessments of program participants and provides therapy as needed;
* Reviews and documents all clinical services and activities;
* Participates in case conferences involving assigned program participants;
* Prepares Discharge Summaries, as needed;
* Assists other WestCare staff as needed to promote cooperation and portray a professional image to the community;
* Embraces and embodies the mission, vision, guiding principles, clinical vision and goals of WestCare Foundation;
* Performs any other duties as assigned.
Qualifications
Essential Qualifications:
Certifications/Licensures:
* Veterans are perferred
* CADC or similar certification preferred; Preference will be given certified candidates;
* Valid driver's license and the ability to obtain and maintain WestCare's vehicle insurance is required.
Education:
* Graduation from an accredited four-year college with a degree in Counseling, Criminal Justice, Social Work, Psychology or other field related to the position is preferred;
* High School Diploma or equivalent is required;
Experience and Competencies:
* Experience working with individuals suffering from mental health and substance abuse disorders required;
* At least two (2) years of experience in counseling, therapy, counseling, and/or social work strongly preferred;
* Ability to define problems, collect data, establish facts, and draw valid conclusions required;
* Excellent communication skills are required;
* Ability to exercise good judgment and discretion required;
* Ability to work well in a team environment required ;
* Adherence to the highest standard of ethical conduct, especially to standards governing confidentiality required;
* Professional appearance and demeanor required;
* Must be culturally/linguistically sensitive to populations served;
* Intake and assessment experience is highly preferred;
* Bilingual ability is preferred;
* Prior experience using office equipment, including phones, scanners, printers, and fax machines, and web cameras is highly desired;
