General Purpose of Job: The On Call Nurse ranks patient's health problems according to urgency, educating and advising patients, and making safe, effective and appropriate dispositions by telephone. The On Call Nurse provides direct nursing care, treatment, and health instructions to home care patients after office hours based on assessment question answers and decides on a solution that will meet the patient's needs, promote the patient's well-being, and maintain a cost-effective standard. Essential Duties: This job description is intended to cover the minimum essential duties assigned on a regular basis. Caregivers may be asked to perform additional duties as assigned by their leader. Leadership has the right to alter or modify the duties of the position.
- Use of a systematic assessment of the patient's symptoms or problem so that a decision that ensures the patient's well being can be made. Determine whether the problem requires medical or nursing intervention and the urgency of that intervention.
- Utilizes assessment results to contact patient's physician for further orders or treatment not covered under the plan of care.
- Perform or reassign unplanned, uncovered, or emergent visits as necessary based on the triage assessment and document in EMR.
- Perform or reassign planned visits that are uncovered per contract
- Communicate events to the agency via electronic record and/or telephone calls prior to the beginning of each business day before 7:30 AM
- Assess patient's condition and initiates appropriate nursing and/or medical care in accordance with established policies.
- Completes required documentation according to Agency policy. Maintains necessary clinical records.
- Maintains "in home office" and electronic devices accordingly to HIPAA regulations.
- Demonstrates and teaches the patient, family members, and/or other caregivers in patient care and treatment procedures during home visit.
- Observes, assesses, and reports to physician and other assigned personnel signs and symptoms of disease process, changes in the patient's physical and/or emotional condition, and patient's response to the therapeutic patient centered goals
- Maintains all equipment, supplies, and environment in a safe manner.
- Maintains knowledge of current trends in nursing and home care.
- Maintains professional standards, compliance with State, Federal (CMS conditions of participation) and Joint Commission regulations, and infection control protocol by participating in in-services, staff meetings, process improvement teams, continuing education seminars when available, and adhering to the SHC expectations - standards of care guidelines.
Job Requirements
General Requirements |
Current RN License in the State of Michigan. Valid driver's license and automobile insurance. American Heart Association BLS Certification upon hire. |
Work Experience |
Minimum of two year acute care or long term care clinical experience. Recent experience in home care or public health (Preferred) |
Education |
Must be a graduate from an accredited school of Nursing. BSN (Preferred) |
Specialized Knowledge and Skills |
Excellent assessment and triage skills. Demonstrated ability exercise independent judgment. Demonstrated interpersonal skills to effectively and appropriately communicate with patients, families, physicians, and interdisciplinary team. Demonstrated understanding of public health principles and practices. Demonstrated ability to document and assemble data in proper format. Ability to enter/exit patient homes that may/may not be handicap accessible. Demonstrates ability to use a keyboard as may be required to perform the essential duties of the job. |
