Assist in the creation and maintenance of a positive and collaborative working environment where all FCT employees and enrolled youth and their families feel welcome, supported, and comfortable. Provide Intensive Care Coordination to enrolled youth and their families as determined by the FCT ICC Director. Provide direct administrative and clinical supervision of up to eight (8) FCT Care Coordinators as they implement the Intensive Care Coordination (ICC) process with enrolled youth and families. Meet in weekly individual supervision with each FCT Care Coordinator assigned to him/her. Assist with the supervision of FCT Family Partners as defined by the FCT ICC Director and FCT Peer to Peer Support Director. In partnership with the Supervisor of Peer-to-Peer Support, co-lead regular group supervision meetings with all FCT Care Coordinators and FCT Family Partners assigned to them. Identify ongoing training needs for the FCT Care Coordinators assigned to him/her and work with the Director of ICC to meet these needs. Assist in the recruitment and orientation of new FCT Care Coordination staff. Review all written work for FCT Care Coordinators assigned. Receive complaints or concerns from enrolled families, providers, state agency representatives, or anyone who expresses concern about the ICC process and works with the ICC Director to remedy such issues. Assist the FCT ICC Director and FCT Peer to Peer Support Director with refinement and development of FCT policies and procedures. Participates as appropriate in all meetings, training, evaluation/research, supervision, quality management, and related activities as may be directed by the FCT ICC Director. Follow all policies and procedures of the agency. Carries an after-hours pager and provides telephonic and, at times, face to face after hours support to youth and families as directed by the Supervisor of Care Coordination. Work assigned hours, which may include some weekend and evening hours. Perform other job-related duties. Respects diverse views and approaches, demonstrates Standards of Respect, and contributes to creating and maintaining an environment of professionalism, tolerance, civility and acceptance toward all employees, patients, and visitors.
Qualifications:
Master's degree in Psychology, Counseling, or Social Work or related field. Previous experience implementing the Wraparound Process as described by the Children's Behavioral Health Initiative and/or National Wraparound Initiative Demonstrated record of successful engagement and formation of positive, functional working relationships with youth and their families Demonstrated record of successful engagement and collaboration with colleagues in the family/child serving human services system including but not limited to clinical providers, primarily care providers, case managers, school personnel, and/or state agency representatives. Current valid US-issued driver's license and ability to provide a registered, inspected, and insured automobile for work related purposes including transporting clients in your own vehicle.
