Position Summary

The Center for Development and Disability (CDD), established in 1990, is New Mexico's University Center for Excellence in Developmental Disabilities Education, Research and Service. These centers, authorized by the Developmental Disabilities Assistance and Bill of Rights Act, build the capacities of states and communities to respond to the needs of individuals with developmental disabilities and their families.

The CDD Family and Community Partnerships Division is seeking to hire an experienced professional to provide education and support coordination to individuals receiving Medicaid Waiver services in New Mexico. The individual hired for this position will help participants navigate the Mi Via and Supports Waiver systems. In response to the growing participation in the Southeast Region of New Mexico, there is a pressing need for candidates who are residents of this area.

This position will provide assistance and guidance with eligibility, certification, pre-planning, service & support plan development, and budget development as part of their assigned caseload.

The role also includes:

  • Conducting monthly visits to see participants in their homes and community
  • Providing education and training
  • Maintaining electronic clinical files in compliance with NM State guidelines

The successful candidate will be working with a dynamic team of professionals to meet the needs of individuals and their families throughout New Mexico. This position requires regular statewide travel.

Conditions of Employment
  • Employees who provide services or work in patient care or clinical areas are required to be in compliance with the University's influenza vaccination requirement.


Minimum Qualifications

Bachelor's degree; at least 3 years of experience directly related to the duties and responsibilities specified.

Completed degree(s) from an accredited institution that are above the minimum education requirement may be substituted for experience on a year for year basis.

Experience

  • working with and supporting individuals with developmental disabilities and their families;
  • in case management and accessing state and community systems, services and resources for persons with disabilities and their families;
  • with self-directed supports and person-centered planning tools leading to the development and implementation of goals and activities.

Bilingual in English and Spanish and strong verbal and written communication skills.

Strong analytic, problem solving, and writing skills.

Outstanding interpersonal skills.

Skilled in utilizing computers and proficient in navigating multiple software programs.



Additional Requirements
This position requires passing a Background Check with Fingerprinting and regular statewide travel.
