Summary: Qualifications *Masters degree in Social Work required and must hold current license from the State of New Mexico as a Licensed Independent Social Worker (LISW) or Licensed Clinical Social Worker (LCSW), plus five years hospital social work or discharge planning experience in relevant setting. *Licensed Master Social Work (LMSW) licensure plus ten years hospital social work or discharge planning experience in relevant setting. Working knowledge/experience in case management. National Case Management or Hospice/Palliative Social Work certification preferred.
