ABOUT THE JOB

The Care Team Associate - Clinical will assist nursing staff in providing direct patient care from the admission process to activities of daily living (ADLs) through patient transfer and discharge.

WHAT IS INTEGRATED MEDICINE?

Integrated Medicine is a 24-bed unit that provides a safe environment for medical and surgical patients who also have behavioral needs. This unit takes an innovative approach to serve patients' complex medical and behavioral needs in one thoughtfully designed location. Extensive safety features such as video monitoring technology and anti-elopement measures are integrated to ensure a safe and therapeutic environment for our staff, patients, and visitors. We aim to remember that we are working in the patient's home away from home rather than having the mindset that these patients are visiting our workspace. RN Staff will be working with Care Team Associates (nurse techs), Milieu Specialists, Therapy Professionals, and dedicated physician staff to provide comprehensive patient care with 4/5:1 RN ratios. Service line supervisors provide 24/7 support to nursing staff. The secure unit has all private rooms, with an open floor plan allowing for patients to socialize and work on community skills with the assistance of our Milieu Specialist. A dedicated Behavioral Health Social Worker assists with meeting the patient's discharge planning needs so that they are able to discharge to the most appropriate and least restrictive environment. Daily interdisciplinary huddles occur to ensure patient care is cohesive and complete.

DETAILED JOB DESCRIPTION

Assist nursing staff /ancillary staff in transferring patients from bed to bed,

stretcher or wheelchair, pickup, and delivery of patient meal trays, and

assisting with patient feeding as directed.

Demonstrate the ability to organize workload and manage multiple tasks while maintaining a clutter-free and orderly environment including prompt answering of unit telephones and accurate delivery of messages and information to patients, families, visitors, staff, and personnel upon request.

Educate and train new staff members in the responsibilities of the Care Team Associate role.

Perform other duties as delegated by nursing staff and unit management as needed in both routine and emergent situations.

Perform patient position changes, placement and removal of patient equipment, daily hygiene care, simple non-sterile dressing changes, finger stick glucose checks, may perform venipuncture to obtain a blood sample, maintain quality control checks, and deliver/retrieve lab specimens and supplies as requested.

Responsible for accurate and timely entry of information into the bed-management system.

Responsible for prompt response to patient call lights and addressing patient needs using positive guest relations skills while ensuring patient vital signs, intake, output, height, and weight are obtained and documented as directed.

May complete unit/department specific duties as outlined in department documents.

REQUIRED QUALIFICATIONS

One of the following:

--Completion of a Fundamentals of Nursing course within 180 days; or

--Certified Nursing Assistant (CNA); or

--Experience with direct patient care such as a medical assistant, nursing

assistant, emergency medical technician (EMT), or surgical technician in the past three (3) years; or

--Completion within the last three (3) years, or will complete within 180 days, an Unlicensed Assistive Personnel (UAP) training program; or

--Nursing licensure candidate who was unsuccessful on the state licensure

examinations in the past three (3) years; or

--Completion a nursing program outside the United States and is awaiting the licensure examination in this country.

Completion of an above qualification's pathway, including patient care experience or a supervised clinical log, within 180 days as a condition of continued employment in this job classification.

PREFERRED QUALIFICATIONS

Additional license/certification requirements as determined by the hiring

department.

PHYSICAL DEMANDS

The physical demands described here are representative of those that must be met with or without reasonable accommodation. The performance of these physical demands is an essential function of the job. This position is very active and requires ambulating, reaching, and moving objects weighing up to and over 20 lbs.

This role also routinely comes into contact with patients who may have

contagious illnesses. This position is in a health care facility open 24 hours a day, 365 days a year. Rotating shifts as well as holiday and work during

inclement weather will be required.
