Summary: Responsibilities: MAIN DUTIES AND RESPONSIBILITIES: Provide intensive clinical and crisis intervention services to children at imminent risk of hospitalization and their families. Includes but is not limited to: Individual psychotherapy family therapy group therapy psychiatric rehabilitation and mental health evaluations. Face-to-face contact with children and their families in their own homes or communities as prescribed in the Individual Treatment Plan. Coordination of services through documented meetings documented telephone conversations and correspondence. Complete assessment and treatment plan in accordance with CFTO standards. Provide emergency evaluations to children and adolescents at imminent risk of psychiatric hospitalization provide crisis intervention services to children at imminent risk of hospitalization in an attempt to stabilize client within the home or community in order to deter hospitalization within a timely manner including documentation of interventions alternatives considered and response of client/client family and supporting agencies. Provide coordinate and monitor treatment services for all assigned CFTO cases. Includes but is not limited to: Face-to-face and telephone contact with client documented in progress notes following established procedures as prescribed in the Individual Treatment Plan. Provision and coordination of care prescribed in the Treatment Plan through documented contact and implementation of assigned activities When providing clinical services arrives and/or participates on time as scheduled. Is alert and responsive to client issues and behaviors. Modifies treatment planning to meet client needs and changes in situation. Documents with client if possible treatment implementation and related issues in progress notes and/or treatment plan review documents. Completing assessment assignments within time requirements specified by procedures. Scheduling/attending treatment planning meetings with staff and other individuals specified in procedures. Completing assessment treatment plan and review documents according to standards. Provide case presentations of client*s assessment at CFTOS treatment meetings. Provide psycho-educational services to children and their families based on modality frequency participant and location specifications prescribed in the Individual Treatment Plan. Services will include provision of information regarding mental illness alcohol and substance abuse effective parenting behavior management etc. Serve as liaison with Bradley Hospital Butler Hospital and other inpatient units as assigned. Liaison activity will include maintaining and documenting contact with client/client family inpatient unit staff and other individuals designated in the Treatment Plan and assisting with Discharge Planning. Case record tasks will be performed following agency procedures including updating of client database reviewing outside correspondence and following client confidentiality and client rights procedures. Provide on-call coverage as assigned. Responds to on-call requests within 10 minutes of receiving page or telephone request responds in person when required within 60 minutes and completes reporting and documentation requirements as specified in procedures. Function as liaison and client advocate with community agencies. This includes but is not limited to: Attending community agency meetings on time. Reporting issues raised at liaison and community meetings to supervisor. Presenting agency policy and procedure statements as required at liaison and community agency meetings and makes other arrangements with other agencies to provide services for identified clients as specified in client Treatment Plans. Participation in committees as designated by supervisor attending meetings on time and actively participating in both discussions and assignments. Participation in other inter-agency meetings and trainings as designated by supervisor attending meetings and trainings on time and actively participating in both discussions and assignments. Provide public education services as assigned. Prepare and attend supervision appointments as scheduled and follow through on assignments and instructions from supervisor. May supervise student interns as assigned. May supervise other clinicians working towards credentialing in same discipline. Other duties and responsibilities as assigned or needed. Other information: QUALIFICATION REQUIREMENTS: Masters* degree in any Human Services field required such as Social Work Rehabilitation or Agency Counseling with three years experience working with adults with serious mental illness plus experience in assessment counseling crisis intervention substance abuse and sexual abuse. NECESSARY SPECIAL REQUIREMENTS: Maintain valid driver*s license at all times. Ability to work evenings on a rotating basis as needed. Ability to communicate with clients and other staff face to face and by telephone. Ability to obtain and maintain C.P.R. certification. Ability to work as a multi disciplinary team member ability to accept establish and maintain working relationships with diverse populations of clients and staff ability to maintain objectivity and ability to maintain perspective under stress; resilience. Maintain valid driver*s license at all times. Ability to work evenings on a rotating basis as needed. Ability to communicate with clients and other staff face to face and by telephone. JOB KNOWLEDGE SKILL AND ABILITY: Ability to read analyze and interpret professional journals and governmental regulations. Ability to write reports and business correspondence. Ability to effectively present information and respond to questions from groups of managers clients customers and the general public. Knowledge of and/or willingness to develop computer literacy skills. Ability to successfully complete all required training and certifications. Oral and written communication skills including speech grammar spelling and legibility are at sufficient level to ensure effective and appropriate communication. Ability to add subtract multiply and divide in all units of measure using whole numbers common fractions and decimals either manually or by use of a calculator.
