Schedule Details:Availability to work 2-3 days per week Monday - Friday, average hours 8:00am -4:30pm with flexibility if needed. Weekend and holiday rotation required. This is a pool/as-needed position offering flexible hours based on the agency's current needs. As our home health agency grows, there may be opportunities for increased hours for larger FTE.

Our highly skilled team provides a wide array of services, including medication education and management, injections and intravenous therapy, wound care, nutritional counseling, bowel and bladder management and much more.Physical, occupational and speech therapists address home safety concerns, strengthening and rehabilitation needs.Our licensed social workers offer assistance with community resources, financial assistance and supportive care and counseling to patients and families.


What You Will Do:

The Occupational Therapist will prescribe, delegate and coordinate the patient plan of care for rehabilitation. The OT establishes a prognosis, and plan of treatment, implementsoccupational therapytreatment for patients based on evaluation results and plan of care. The OT is responsible to re-assess the ongoing effects of treatment, collaborate with and provide consultation to others in care delivery.This profesisonal will alsosupervise occupational therapy assistants.

What You Will Need:



  • Bachelors of Occupational Therapy
  • At least 6 months in home health setting preferred.
  • Valid State of WI Driver's License with acceptable driving record required.
  • Current BLS certification through AHA(or ability to obtain at time of hire).
  • Occupational Therapist licensure required through State of WI (OTR).
