Assessment Provide telephonic screening of BH members using PHQ-9, GAD, and other CM-DM diagnostic tools to determine additional care support needs. Communication Communicates with HMSA members, physicians, case managers, and disease management team members. Coordinate with other colleagues to ensure continuity of care for members in assigned work queue. Triage After assessment, triage members to appropriate BH program Data Management and Documentation Complete and file forms and input data into appropriate HMSA systems. Education Provide navigation, referrals to community resources, and educational materials. Performs all other miscellaneous responsibilities and duties as assigned or directed #LI-Hybrid
