UNM Medical Group, Inc. is hiring for a Counselor Social Worker I or II to join our Behavioral Health Clinic. This opportunity is a part-time (0.5FTE 20 hours a week) day shift opening (Wednesdays, Thursdays, and partial days on Fridays) that is 100% on-site in Rio Rancho, New Mexico.

Counselor Social Worker I Minimum $52,915 - Midpoint $66,185*

Counselor Social Worker II Minimum $61,776 - Midpoint $77,251*

*Salary is determined based on years of total relevant experience.

*Salary is based on 1.0 FTE (full time equivalent) or 40 hours per week. Less than 40 hours/week will be prorated and adjusted to the appropriate FTE.

Summary:

Provides, oversees, and/or administers a wide range of psychosocial evaluation/assessment, crisis intervention, and/or case management services in a high-intensity clinical environment which requires a high degree of independent decision-making and program administration.

Minimum Job Requirements of a Social Worker I:

Master's degree in Counseling, Social Work, or a related degree. No experience required. Licensed eligible Counselor or Social Worker in the State of New Mexico, or eligible for license in the State of New Mexico required. Completed degree from an accredited institution that are above the minimum education requirement may be substituted for experience on a year for year basis. Verification of education and licensure will be required if selected for hire.

Minimum Job Requirements of a Counselor Social Worker 2:

Master's degree in Counseling, Social Work, or a related degree with 1 year directly related experience. Licensed Independent Social Worker (LISW) OR Licensed Clinical Social Worker (LCSW) OR Licensed Professional Clinical Counselor (LPCC) OR Licensed Mental Health Counselor (LMHC) licensure pending, as documented by temporary license. BLS certification required. Completed degree from an accredited institution that are above the minimum education requirement may be substituted for experience on a year for year basis. Verification of education and licensure will be required if selected for hire.

Duties and Responsibilities of a Counselor Social Worker I:



  1. Facilitates client access to community resources, including locating recreational activities, housing, food, clothing, school programs, vocational opportunities or services, and relevant mental health services; assists client to develop natural resources and make contact with social support networks.
  2. Provides information, education and coaching in life skills (e.g. wellness promotion, parenting skills, job-search strategies, social skill building, and brief interventions) for clients and their families in a compassionate, professional and culturally responsive manner. Will also provide health literacy training as well as disease management skills.
  3. Engages clients in a process of ongoing information gathering and decision making in order to empower the client to identify their own goals, strengths and challenges.
  4. Serves as liaison, coordinator and/or advocate on behalf and in collaboration with patients to effectively transition care to routine primary care.
  5. Provides brief intervention to clients and families as appropriate to the position; contribute to that development of treatment plans, discharge plans, and documentation for coordination of care, follow-up care programs; provides therapeutic crisis intervention and emergency services as required.


Duties and Responsibilities of a Counselor Social Worker II:



  1. Provides counseling, therapy, and/or psychotherapy to clients and families as appropriate to the position; prepares treatment plans, discharge plans, and follow-up care programs; provides therapeutic crisis intervention and emergency services as required.
  2. Collects data about patients through interview, case history, psychological tests, and/or observational techniques; evaluates data to identify causes of problems and to determine proper therapeutic approach or referral to other specialists.
  3. Consults with other therapists and related professional and paraprofessional staff, as appropriate, in the performance of therapeutic and/or casework; refers clients to appropriate service agencies as required.
  4. Provides and/or arranges for therapeutic interventions as appropriate for patients or clients in a crisis condition and for those with serious disturbance problems.
  5. Refers clients to appropriate social service agencies for financial assistance and other required services; may visits patients and families in their homes to provide counseling and outreach services.



About the Behavioral Health Clinic - Rio Rancho:

The Behavioral Health Clinic has an interdisciplinary team providing accessible behavioral health care to the Sandoval County community, while also coordinating care with integrated behavioral providers at the neighboring Sandoval Regional Medical Center and nearby Grande Primary Care Clinic. Team members work collaboratively across behavioral health and related disciplines, including psychiatry, psychology, nursing, primary care, clinical social work, counseling, and peer support.
