Wage - $22.00 - $25.00 Schedule - Monday - Friday, 8 am to 5 pm Benefits * Medical/Dental * FSA * 403(b) Retirement * Additional Voluntary Benefits * Paid Holidays * Vacation/Sick Leave * Education Assistance Program * Employee Assistance Program * Longevity awards Position Summary: The ECLIPSE Home Visitor is responsible for home visits and case management of children and families enrolled in the program, participating in assessment, treatment planning and multidisciplinary meetings and connecting children and families with resources and services. Responsibilities: Participate in intake assessment completed by the Mental Health Consultant with caregivers and child.
Provide ongoing case management and manage a caseload of ECLIPSE children and families.
Facilitate multidisciplinary meetings with parents, sources of referrals and allied service providers to establish a treatment plan for the child.
Complete all documentation according to agency policy and required by the contract.
Attend and participate in staff meetings, trainings, and individual and group supervision.
Observe confidentiality in all related program functions with staff and families. Organize and participate in family events and parent meetings. Perform other duties as assigned Qualifications

Physical Requirements: This position normally requires the physical demands of standing, walking, bending, lifting or performing other work requiring medium physical exertion, talking and hearing and adequate vision on a regular basis to perform the job requirements. These physical demands are required up to 60% of the time. In addition, approximately 40% of the time may include lifting and carrying children, moving quickly around the room to maintain appropriate supervision and safety and sitting on the floor or in small chairs with the group.
