POSITION DESCRIPTION

POSITION TITLE: Case Aide

LOCATION/DEPT: Pathway Forward Program

REPORTS TO: Program Manager - Operations

FLSA CATEGORY: Non-Exempt

STATUS TYPE: RFT

DEPARTMENT MISSION

The Case Aide assists the Re-Entry Case Managers (RCM's) in meeting their case management requirements by preparing and compiling records, assist in maintaining case files, scanning of documents into electronic case management files, medication review.

The Case Aide assists the RRC program by conducting weekly orientation groups, mail sorting/distribution, securing, inventorying, & returning resident's property, may be asked to conduct timely self-reported paperwork (i.e. medical screens, filling out spot check forms, etc.) act as a back up to the Discipline Coordinator and assist in the Urinalysis Department as needed.

OUTCOMES

Sufficient administrative support will be provided to ensure that program duties are effectively achieved. This includes assistance with planning, communications, and data management.

ESSENTIAL DUTIES AND RESPONSIBILITIES



  1. Gather case file documentation.
  2. Ensures prompt scanning of case file documents into electronic case files.
  3. Always ensure resident confidentiality.
  4. Conduct weekly orientation groups for residents.
  5. Responsible for inventorying, securing, and disposal of residents' property according to policy.
  6. Develop the weekly PRT list and enter resident level changes in SecurManage.
  7. Attend RCM meetings, staff meetings, and all staff trainings.
  8. Properly stores and inventories all closed out files along with file retention.
  9. Retrieval of documents from the control desk(s).
  10. Enter visitor lists into SecurManage.
  11. When needed, escort medically complicated residents to their appointments.
  12. Assist in the obtaining furlough signatures.
  13. Assist with entering passes for residents on behalf of RCMs under supervision.
  14. All other duties as assigned.


REPORTING RELATIONSHIPS

This position reports to the Program Manager - Operations.

In contacts related to this position's duties, this individual acts as a representative of The Salvation Army and its mission.

PERFORMANCE MEASUREMENTS

This individual will be evaluated on how effectively the outcomes of this position are achieved, as well as the timeliness, accuracy, and completeness of performance.

A performance review will be conducted.

EDUCATION/EXPERIENCE



  • An Associate's degree in criminal justice, social services, or a related field is required.
  • Two years of experience in a criminal justice or social services environment is required.

POSITION LIMITATIONS



  • This individual will only commit Army resources that have been allocated or approved.
  • This individual will keep the Program Manager - Operations informed on all critical issues relating to their area of responsibility.
  • This individual will adhere to all policies and procedures in carrying out the responsibilities of this position.


PHYSICAL DEMANDS/WORK ENVIRONMENT



  • This position is required to do light physical work and must be able to lift 10 pounds.
  • In order to successfully perform the essential functions of this position, the employee is regularly required to use standard office equipment and a PC.
  • The work environment for this position is a residential environment with a low to moderate noise level.
  • Maintain positive work atmosphere by behaving and communicating in a manner that fosters and maintains positive relationships with co-workers and supervisors.
