The Advanced Practice Provider exercises independent judgement in the assessment, diagnosis and initiation of medical processes and procedures, in accordance with the licensure regulations of the Commonwealth of Virginia, and as delineated in the written Scope of Practice and Protocol. Demonstrates respect for and an understanding of other clinical disciplines while using an integrated team-based approach to achieve desired patient outcomes. Utilizes evidenced based standards, guidelines and protocols for care delivery. Participates in quality and best practice initiatives by utilizing standards and guidelines for cost effective care delivery to continuously improve patient care and enhance outcomes. Performs high-quality clinical practices and effective patient management under physician supervision by following state regulations, including primary and secondary specialty care in clinical and surgical practice settings. Inova Medical House Calls is looking for a Nurse Practitioner to support our communities in Northern Virginia. The Medical House Calls program brings primary care into the homes of adults over the age of 65 who cannot drive and reside within the program's extensive 52 zip code coverage area. This initiative is driven by an interdisciplinary team that delivers comprehensive primary care directly to patients in their homes or assisted living facilities. Typically serving patients aged 65 and above who face challenges in attending traditional medical appointments, the program primarily assists individuals managing multiple chronic conditions, coping with changes in functional ability, handling polypharmacy, and dealing with frailty or debility. The primary care services provided through this program involve intensive patient management and meticulous care coordination, with a particular focus on advance care planning. To ensure optimal patient outcomes, the program team establishes robust partnerships with various healthcare entities, including inpatient teams, hospice agencies, skilled nursing facilities, physical therapy, occupational therapy, mental health services, and county support services. The geographical coverage of Medical House Calls extends across approximately 48 zip codes in Northern Virginia. Medical House Calls | Inova Job Description Provides safe and effective care to ambulatory patients under the supervision of a collaborating physician. Utilizes independent judgment in the assessment, diagnosis, treatment planning, and provision of ongoing care to ambulatory patients that is in accordance with nurse practitioner licensure regulations for the Commonwealth of Virginia and delineated in the written Scope of Practice/Protocol for Nurse Practitioner. Teamwork/Coordination of Care: Demonstrates ability to communicate effectively and work collaboratively with others. Exhibits respect for and understanding of other clinical disciplines and uses an integrated approach to achieve desired patient outcomes. Evidence-Based Practice/Learning: Uses evidence-based standards, guidelines, and protocols to the delivery of patient care. Incorporates new information and approaches, when appropriate, to continuously improve patient care and practice. Professionalism / Professional Development: Ensures ongoing growth and development of clinical skills and knowledge by assuming responsibility for lifelong learning.
