What Sets Us Apart AI-powered documentation support that simplify documentation and help you stay focused on patient care Meaningful 1:1 patient relationships in the home Greater autonomy in managing your patients and plan of care 4 weeks of paid clinical orientation to support your transition into home care 31 days of paid time off plus 9 paid company holidays Tuition reimbursement, CEU credits and career development opportunities What You'll Do Provide personalized nursing care in patients' homes as part of an interdisciplinary care team Develop and manage individualized plans of care Assess changing patient needs and adjust care accordingly Educate and support patients and caregivers throughout the care journey Use available clinical technology, including AI-powered documentation support, to support efficient and accurate Qualifications What You'll Need Licenses and Certifications: License and current registration to practice as a registered professional nurse in New York State required. Valid driver's license or NYS Non-Driver photo ID card may be required as determined by operational/regional needs. For Hospice Only: Wound Care Certification required or willingness to obtain within one (1) year of hire preferred. Certification in Hospice and Palliative Care Nursing preferred. Education:Degree in Nursing from an approved program required. Bachelor's Degree - NFP Only: Nursing required. Work Experience:CHHA: Minimum of one year of experience in a medical/surgical environment required. Hospice: Minimum of one year of clinical nursing experience, including at least six months of Hospice, Med/Surg, ICU, Emergency Department, Home Care, Palliative Care, Oncology or Long Term Care experience required. NFP: Minimum of one year of recent experience in maternal/child health required. Demonstrated basic computer skills required.
