JOB SUMMARY Provides coordinated care in the home to clients and assists with creating patient centered action plans to promote effective education, self-management support, and adherence to the PCP provider's plan of care facilitating access and timely healthcare delivery. EXPERIENCE AND EDUCATION Required
Master's Degree in social work
1 year of experience in a health care setting preferable with homebound patients.
Current license is required by the Texas State Board of Social Work Examiners. (DRIVERS) DRIVERS LICENSE TX Driver's License and proof of driver's insurance JOB DUTIES
Salary is commensurate with education, skills, and relevant experience.
