JOB SUMMARY

The Intake Coordinator Mental Health Specialist Case Manger IV reports to the PSC Supervisor and is responsible for completing intake process with new client referrals and providing case management and rehabilitation services to individuals. This will include assisting individuals in gaining access to Medi-Cal, primary care linkage, social support, financial support, housing options, employment services, educational support, and other services as needed.



ESSENTIAL JOB RESPONSIBILITIES



  1. Under the supervision of the Supervising Personal Service Coordinator, assist in the accomplishment of the case management plan with clients by screening, evaluating, identifying, and implementing specific case management activities as identified in the treatment plan to support clients in achieving their goals.
  2. Plan, coordinate, andfacilitateservices for Severely Mental Ill (SMI) clients.
  3. Work on a multi-disciplinary team to support clients with an array of needs delivered as an integrated service, including, but not limited to, medications, therapy, and groups, housing, employment, benefits, transportation, physical health, and substance use.
  4. Provide frequent and assertive contact to individuals, primarily community/ field-based services with occasional office-based services.
  5. Provide direct services, including case management and rehabilitation skills to clients. Such services may include group counseling, individual,and/orother forms of direct service as outlined in the Plan of Care.
  6. Interacts with program staff to schedule intake assessments.
  7. Respond to referring parties upon receiving referrals, meeting with clients, assessing desire for program participation, completing intake forms and coordinating/scheduling appointments upon enrollment.
  8. Develop and maintain outside supportive referral services for clients.
  9. Attend daily team meetings to consult and coordinate client care.
  10. Be available to respond to crisis in the community.
  11. Attend weekly supervision meetings with the Supervising Personal Service Coordinator.
  12. Maintain andsubmitall documentation and billing as it relates to client services ina timelyfashion.
  13. Meet monthly key performance metrics, including end of the month deadlines, productivity standards and documentation timeliness expectations.
  14. Communicate closely, in a systematic fashion, with agencies and significant others involved in the rehabilitation of clients.
  15. Provide the Supervising Personal Coordinator withtimelyreports on any unusual occurrences that mightimpactthe overallprogramoperation or the health and safety of clients or employees.
  16. Think and act quickly and efficiently in emergencies.
  17. Complete training requirements set by the agency and county.
  18. Will drive on agency business, either in a company vehicle or personal car, including picking up, transporting and dropping off clients.
  19. Physical presence at the program is required.
  20. Employees are expected to manage their cell phone use so that the Agency cell phone stipend covers business-related usage related to two-factor authentication (2FA) and required HRIS access.



ESSENTIAL JOB REQUIREMENTS



  1. Abachelor's degreefrom an accredited or regionally approved college or universityinsocial work, psychology, vocational rehabilitation, criminal justice, or a related field that isdirectly relatedto the specific responsibilities of the job.
  2. At least one year of experience in a mental health or human services related field.
  3. Current certified training in emergency first aid procedures preferred.
  4. Knowledge of best practices regarding the treatment of clients with Severe Mental Illness (SMI) and Dual-Diagnosis individuals.
  5. Communicate effectively in written and spoken English.
  6. Intermediate computer skills in Office 365, Teams, Zoom and other communication/meeting platforms.
  7. Possess a valid driver's license and have access to a dependable means of transportation that is properly insured and operated in accordance with all laws. Agency insurer authorization is required.
  8. Ability to pass a pre-employment criminal background check.
  9. Ability to pass a pre-employment drug screen, and TB test.



ESSENTIAL JOB REQUIREMENTS FOR BILINGUAL SKILL



  1. Communicate effectively in written and spoken Spanish or Hmong.
  2. Required ability to communicate effectively with monolingual Spanish or Hmong speaking clients andfamiliesfor all clinical purposes.
  3. Assistwith phone calls for Spanish or Hmong speaking clients.
  4. Required to provide language translations for case managers and Physicians on a periodic basis.
  5. Minimal adequate competency in the Spanish or Hmong language asevidencedby successful passage of the TPOCC Spanish Competency Test.
  6. Other translation tasks asrequired.

    • Bilingual: Class 140 ($23.34 - $30.72/hour)
