Overview
Schedules visits using Home Care Home Base and communicates with field staff, patients, physicians, etc. to maintain proper care coordination and continuity of care. Performs clerical, customer service, data entry and processing, and coordination activities which support patient service delivery. Works under direct supervision. Maintains patient schedules according to prescribed visit frequency & duration in the Plan of Care (POC). Enters any requested follow-up changes to patient schedule as directed by a licensed clinician/ manager to ensure compliance. Assigns & reassigns visits to clinicians following VNS Health Home Care standard operating procedure (SOP) guidelines to maximize staffing resources and workload balance across team clinicians. Notifies RN's, Rehab & other clinicians of daily assignments and of any changes in schedules to enhance patient safety and satisfaction. Processes clinician unplanned PTO according to SOP guidelines in a timely manner. Completes requested schedule tasks according to back-office workflow guidelines in the VNS Health Home Care Scheduling SOP. Requests include the following types of visits: Skilled Nursing, Physical Therapy, Occupational Therapy, Speech-Language Pathology, Licensed Social Worker, , if appropriate. Reviews service codes by visit by discipline and checks to see appropriate buddy codes are set up in advance for specific visit needs, such as supervisory visits, wound measurements, or other visit specific tasks to ensure compliance with VNS Health Home Care policy and regulatory mandates. Completes administrative tasks assigned to the Scheduler role including, but not limited to, generating required reports such as Agent Summary Report, Unverified visits report etc. and reconciles reports in accordance with VNS Health Home Care Scheduling SOP's. Prepares and reviews clinician (i.e., worker) schedules daily to identify productivity or geographical services issues. Communicates schedule concerns to Clinical Field Manager (CFM) and/or Branch Director in a timely manner. Processes declined, missed, rescheduled and reassigned visits in workflow according to VNS Health Home Care SOP guidelines in a timely manner. Answers calls from the field staff and refers clinical questions to branch management as necessary. Reviews, processes and completes coordination notes as part of VNS Health Home Care SOP workflow in a timely manner. Escalates unresolved issues to Branch Management in a timely manner. Adheres to, participates in and completes the VNS Health mandatory HIPAA and Employee Compliance Program. Participates in special projects and performs other related duties, as required. Qualifications Education: Associate degree in related field OR equivalent work experience required. Experience: Minimum of two years of administrative/general office experience, which includes heavy customer interaction, required. Demonstrated ability to allocate resources negotiate with team members to reach win-win results required. Proficient personal computer skills, including MS Word, Outlook, and Excel required. Effective oral/written communication skills required. Bilingual skills may be required, as determined by operational needs. Pay Range USD $28.60 - USD $28.60 /Hr.
