About the Job

Community-University Health Care Center (CUHCC) is seeking a compassionate and experienced Mental Health Practitioner to join our Adult Rehabilitative Mental Health Services (ARMHS) team. This position provides individualized, recovery-oriented services to adults with serious mental illness (SMI), helping clients develop and restore skills that support psychiatric stability, independent living, social functioning, community integration, and overall well-being.

Services are provided in clients' homes, community settings, at CUHCC, and other appropriate locations. The role offers the opportunity to work independently while collaborating with an
interdisciplinary team to support clients in achieving their recovery goals.

Salary Range: The salary range for this position is between $23.84/hr - $29.27/hr, which equates to between $49,587.20/annually - $60,881.60/annually for 1.0 FTE, depending on qualifications.

This position reports to the Community Mental Health Program Manager (ARMHS and DVSA) and has no direct reports.

Work Schedule & Type of Position: The work schedule will generally be Monday through Friday during daytime hours, with some early evening availability. This position follows a hybrid work model, with 2 days working on-site and 3 days working remotely and/or in the community. Employees should also be prepared to work on-site on additional days when requested or when business needs require.

Please note, this position is not eligible for H-1B or Green Card sponsorship. This position does not offer a STEM OPT training program.

Responsibilities

Clinical Assessment/Rehabilitation - 60%

  • Provide individualized, recovery-oriented rehabilitative mental health services to adults with serious mental illness, focusing on symptom management, psychiatric stability,
    independent living, social functioning, and community integration.
  • Complete functional and level-of-care assessments within scope of practice and develop, review, and update individualized treatment plans based on client strengths, needs, and recovery goals.
  • Provide person-centered and culturally responsive rehabilitative interventions, including skills training, client and medication education, independent living and social skills development, and community-based support.
  • Monitor changes in client functioning, needs, and risk; communicate significant concerns to the treatment supervisor and care team; and assist with crisis prevention, intervention, and stabilization when needed.
  • Provide individual and group services in clients' homes, community settings, and other approved locations while independently managing scheduling, travel, caseload, and productivity expectations.

Administration/Documentation - 25%

  • Complete timely and accurate ARMHS documentation, including progress notes, assessments, treatment plans, treatment plan reviews, and other required documentation in accordance with CUHCC, DHS/MHCP, and payer requirements.
  • Ensure services are medically necessary, rehabilitative in nature, consistent with the client's treatment plan, and provided within the employee's scope of practice.

Care Coordination/Other - 15%

  • Collaborate and coordinate care with multidisciplinary and community partners to promote continuity of care and support client goals.
  • Participate in required clinical and administrative supervision, case consultation, team meetings, training, and field evaluation activities.
  • Complete required ongoing mental health and ARMHS training and maintain applicable competencies.
Qualifications

Required Qualifications

  • BA/BS in psychology, social work, counseling, or a related field plus at least two years of relevant experience, or a combination of related education and experience totaling at least six years.
  • Experience working with individuals with serious mental illness or similar populations.
  • Social Work licensure is required for candidates with a Social Work degree, in accordance with Minnesota Board of Social Work requirements; otherwise, licensure is not required.
  • Valid Minnesota driver's license, reliable vehicle, and required insurance for travel to client homes, community locations, and other service sites.
  • Effective written and verbal English communication skills and proficiency with electronic health record documentation.
  • Ability to work independently in client homes and community settings while maintaining appropriate safety, professional boundaries, confidentiality, and sound judgment.

Preferred Qualifications

  • Master's degree in social work, psychology, counseling or related field.
  • Strong communication and interpersonal skills with the ability to build rapport with individuals from diverse backgrounds.
  • Experience providing ARMHS, community-based mental health, psychiatric rehabilitation, case management, or similar services to adults with serious mental illness.
  • Experience developing and implementing individualized treatment plans.
  • Experience working in a community mental health or integrated healthcare setting.
  • Proficiency in one of CUHCC's commonly spoken languages, including Spanish, Somali, Vietnamese, Hmong, Pashto, or Dari.

This position requires a background check, a negative TB test, and a Hepatitis B vaccination.

At CUHCC, we're transforming urban health care and educating the next generation of healthcare professionals.

CUHCC offers a full range of dental and mental health services that are typically not offered at a community health center, such as prosthodontic services and psychiatry. CUHCC has pharmacists, psychiatric nurse practitioners, dentists, dental hygienists, nurses, family nurse practitioners, family practice physicians, internal medicine physicians, internal medicine-pediatric physicians, licensed social workers and psychologists among other disciplines on staff.
