Lutheran Social Services (LSS) is a private, non-profit, human service agency providing a variety of services throughout Wisconsin and Upper Michigan. Over the years, LSS has grown to become one of the largest human service agencies in the two state area serving more than 125,000 clients annually. Service Facilitator - CCS

Comprehensive Community Services (CCS) Program

Location: Trempealeau County, WI

Schedule: Full-Time, 40 hours per week



Make a difference by helping individuals and families access the support they need to thrive.

LSS is seeking a compassionate and organized Service Facilitator to partner with individuals, families, and community providers to assess needs, develop personalized service plans, and coordinate services that promote independence, well-being, and growth.



Compensation based on education and/or credentials:

Bachelors: $24.20/hr

Master's Degree: $27.20/hr

Master's w/therapy licensure: $28/20/hr


What You'll Do

  • Conduct functional and needs assessments.
  • Develop and implement person-centered service plans.
  • Coordinate services and connect clients with community resources.
  • Facilitate team meetings and collaborate with families, providers, and support networks.
  • Maintain timely, accurate documentation and client records.
  • Advocate for clients and support positive outcomes through trauma-informed, strengths-based practices.


What We're Looking For

  • Bachelor's degree in Social Work, Psychology, Human Services, Counseling, Sociology, Child Development, or a related field.
  • Experience working with individuals, families, or vulnerable populations preferred.
  • Strong communication, organization, and relationship-building skills.
  • Ability to manage documentation, coordinate multiple services, and work independently.
  • Valid driver's license, reliable transportation, and ability to travel within the community.
