Position Highlights
- Provide comprehensive primary care services for adult patients and families
- Deliver preventive, acute, and chronic disease management
- Partner closely with Advanced Practice Clinicians and specialty colleagues
- Utilize evidence-based medicine and population health strategies
- Manage a full continuum of care while developing long-term patient relationships
- Participate in a collaborative care model designed to improve quality and outcomes
Primary Responsibilities
- Diagnose and treat acute and chronic medical conditions
- Conduct patient assessments and develop individualized treatment plans
- Review medical histories, medications, laboratory results, and diagnostic studies
- Order and interpret laboratory, imaging, and other diagnostic tests
- Coordinate care with specialists and ancillary services
- Promote preventive care and wellness initiatives
- Ensure high-quality, patient-centered care across the continuum
Qualifications
- MD or DO degree
- Board Certified or Board Eligible in Family Medicine or Internal Medicine
- Unrestricted New York medical license or license application in process
- Active DEA license or ability to obtain prior to start date
- Commitment to delivering high-quality, value-based care
Compensation & Benefits
- Compensation range: $238,500 - $392,500
