Title: Resource Specialist

Duration: 3 Months - Possible extension

Location: Remote FL -
Region A ( Counties Preferred: Santa Rosa, Escambia, Okaloosa, Walton, Holmes, Washington, Bay, Jackson- but these are also counties in Region A: the Tallahassee part: Calhoun, Gulf, Liberty, Franklin, Wakulla, Leon, Gasden, Jefferson, Taylor, Madison, dixie)

Mon-Fri - 8A-5P Eastern time (7A - 5P CT)

Day-to-day responsibilities of this role:

This position will be field-based and will provide care management to assist with coordination of care, including assessing health care needs, referring to health plan providers, providing education on benefits/expanded benefits, and providing community resources to address social determinants of health needs.

The caseload is 100 members. This position will document in an electronic health record and will be required to document outreach notes, complete assessments, and complete a care plan on each member.

Provides resource support to members and connects members to local resources for various services (e.g., employment, housing, independent living, justice, foster care) based on care/service assessment and plans. Works with leadership to identify the best, most appropriate local resources for member's needs based on industry best practices.

Performance expectations/metrics:

  • Compliance with care management policies/procedures including outreach and documentation expectations.
  • Individuals will be audited on a monthly basis to ensure charts are in compliance.


First day looks like:

  • The first day will align with training/onboarding. The first day will consist of setting up computer, completing onboarding tasks, reviewing training schedule, and meeting with the training team. Onboarding is 4 weeks.

1. Pediatric case management experience 1. Health plan experience
2. Experience connecting families/children to local resources for various services including employment, housing, independent living, justice, foster care, etc. 2. Foster care and/or adoption experience

Education Requirement: Bachelor's Degree Education Preferred:
Software Skills Required: Microsoft Office- Outlook, Word, and Excel
