Cognitive Care Navigator
Qualifications
Make a meaningful difference in the lives of older adults. The Opportunity As a Cognitive Care Navigator, you will support individuals experiencing memory loss, cognitive changes, dementia, and related conditions, along with their care partners and families. You will help them understand their options, access appropriate resources, and feel supported before, during, and after diagnosis. You will provide cognitive care navigation, coordinate memory-screening initiatives, support caregivers, and contribute to community education and outreach. Working with Adult Day Services, Clinical Intake, Care Management, and other interdisciplinary teams, you will help promote early identification and coordinated support for individuals and families navigating cognitive change. Masters of Social Work (required)
Experience
3 years: A minimum of 3 years of experience with older adults, caregivers, or individuals with dementia or related conditions is required. Experience in counseling, care coordination, navigation, caregiver support, assessments, and resource referral is required.
