Make an impact at the front door of healthcare. Bring your clinical judgment beyond the bedside and help patients navigate pivotal transitions in care while supporting safety, access, and efficient patient flow. As an Emergency Department RN Case Manager at UVA Health, you will combine your acute care expertise with real-time assessment, discharge planning, and care coordination. Working alongside emergency physicians, nurses, social workers, rehabilitation providers, and community partners, you will help patients receive the right care, at the right time, in the right setting. The Role This is a highly collaborative nursing role for an experienced clinician who excels at pattern recognition, critical thinking, and navigating complex care needs. You will assess patients early in their Emergency Department journey, identify clinical and social barriers, and develop safe, practical plans that support continuity of care across settings. Your average day will include:
The Emergency Department offers variety, complexity, and continuous learning. No two days are the same, and your expertise will directly influence discharge planning, healthcare access, patient experience, and system-wide care delivery. This opportunity also offers a primarily weekday schedule with more predictable hours than many traditional bedside roles. A weekend rotation is included, with scheduled time off surrounding worked weekends. MINIMUM REQUIREMENTS Education: Graduate of accredited nursing program. Bachelor's of Science in Nursing degree required within 5 years of hire or transfer into Case Management role. Experience: 2 years of relevant experience required Licensure: Licensed to Practice as a Registered Nurse in the Commonwealth of Virginia required. PHYSICAL DEMANDS Job requires standing for prolonged periods, frequently walking, bending/stooping. Proficient communicative, auditory and visual skills; Attention to detail and ability to write legibly; Ability to lift/push/pull 20-50 lbs. May be exposed to chemicals, blood/body fluids, and infectious disease. The pay range for this role is $39.10 - $51.94 hourly.
