Responsibilities
Spring Mountain Treatment Center is a 110-bed mental health facility providing acute crisis stabilization services for adults, teens, and children in a secure inpatient setting since 2001. Our individualized treatment includes a structured, therapeutic environment, intensive physician involvement and oversight, and daily group and educational programs. We provide a caring and healthy environment that gives individuals a chance to make positive changes. We are currently seeking a dynamic professional to join our team as a Care Navigator. This position is responsible for meeting with all eligible patients to discuss the benefits of Outpatient services as part of their continuum of care treatment. Communicates eligibility to key members of senior leadership and associated departments. Responsible for routine collection and data entry, specific to Outpatient eligibility into Midas for trending report purposes. Have thorough understanding of Step-Down Playbook, implementing components as needed and workflow process as it pertains to case management and discharge planning preferred.
