Description
In this impactful role with UCLA Health the Transplant Coordinator will provide expert, patient-centered clinical case management throughout the transplant journey, from initial referral through ongoing follow-up. This role requires advanced knowledge of organ transplantation and serves as a key liaison among patients and families, physicians, medical staff, referring providers, insurance partners, and organ procurement organizations. The Coordinator ensures seamless interdisciplinary care, provides individualized education and support, facilitates donor and recipient coordination, and promotes quality, resource stewardship, and exceptional patient outcomes. Additional responsibilities include clinical data collection, development of educational programs and clinical protocols, and support of community education and outreach. The Transplant Coordinator reports directly to the Nurse Manager, with clinical functions supervised by the Transplant Program Medical Directors and Nurse Manager. Performance is evaluated jointly by the Transplant Program Medical Directors and Nurse Manager. TC2: must have CCTC
