The MGH Cancer Center is expanding the Early Detection and Diagnosis (EDDx) Nurse Navigator team. Working collaboratively with the physician, APP and administrative team, the EEDx Nurse Navigator is the first point of contact for patients referred to the EEDx clinic for cancer screening, early detection and genetic testing and surveillance. -The Nurse Navigator must have the ability to guide patients through the process of this screening and across the continuum of care. -The qualified candidate with have foundation knowledge in cancer and strong and compassionate communication skills. -The Nurse Navigator has a strong collaboration with both the clinical and administrative team. Summary - Responsible for coordinating the complex care needs of patients and is the primary patient contact for service access, diagnostics, treatment activities, schedules, patient and family education, and multidisciplinary collaboration. Does this position require Patient Care? Yes Essential Functions -In collaboration with the interdisciplinary team, help create a tailored health care plan for assigned patients. -Develop an individualized plan with the MD for appropriate services for the patient. -Reports pertinent observations and reactions regarding clients to the appropriate person (i.e., primary care provider, social worker, or Director). -Assesses and coordinates patient's discharge planning needs with members of the healthcare team. -Resolves patient problems and needs by utilizing multidisciplinary team strategies. -Educates patients regarding how to navigate throughout the health system.
