Description

La Causa Inc. Social Services is dedicated to empowering youth and young adults with complex mental health and developmental needs, along with their families, through compassionate, coordinated, and person-centered care.

We are seeking a dedicated and collaborative Care Coordinator to join our multidisciplinary team. In this role, you will provide comprehensive care coordination for youth and young adults with high-level needs who are dually diagnosed with mental health and developmental disabilities. You will work closely with individuals and their families to develop and implement individualized plans that reflect their unique strengths, goals, and vision for a life that is happy, healthy, and safe. Through partnership, advocacy, and a recovery-oriented approach, you will help connect individuals to the resources and supports needed to achieve meaningful and lasting outcomes.

Your Role:

As a Care Coordinator, you will serve as a central point of support for youth and families navigating complex systems of care. You will lead the development of individualized service plans, coordinate services across providers, and ensure that care is aligned with each individual's goals, strengths, and needs.

What You'll Do:
  • Lead Care Planning - Facilitate the development and ongoing review of individualized, strength-based service plans with youth and families.
  • Coordinate Services - Connect individuals to mental health, developmental disability, medical, and community-based supports.
  • Monitor Progress - Track outcomes, adjust plans as needed, and ensure services remain effective and goal-oriented.
  • Advocate for Clients - Support youth and families in navigating systems and accessing appropriate services and resources.
  • Promote Collaboration - Work closely with therapists, peer specialists, service providers, and families to ensure coordinated care.
  • Support Crisis Planning - Assist in developing proactive strategies to manage and respond to crises.
  • Ensure Compliance - Maintain accurate, timely documentation in accordance with agency, state, and program requirements.
  • Build Relationships - Foster trusting, respectful relationships with youth, families, and team members.
  • Mandated Reporting - Fulfill all responsibilities as a mandated reporter.
  • Professional Development - Participate in supervision, training, and team meetings.
What We're Looking For:
  • Bachelor's degree in Social Work or related field (Required)
  • Master's degree in Social Work or related field (Preferred)
  • At least one (1) year of experience working with youth with mental health and/or developmental disabilities
  • Experience in care coordination, case management, or community-based services preferred
  • Strong cultural competency and interpersonal skills
  • Bilingual (Spanish and English), spoken and written (Highly Desirable)
  • Excellent verbal, written, and documentation skills
  • Strong organizational and time management abilities
  • Proficiency with Microsoft Office Suite
  • Must successfully complete and pass all background checks, including an annual influenza vaccination
  • Valid Wisconsin driver's license, reliable transportation, and state minimum auto insurance

Work Environment:
  • Work occurs in both community and office settings (local travel required)
  • Flexible schedule with no weekend shifts and limited evening hours, based on family needs
Physical Demands:
  • Regular driving, standing, sitting, walking, bending, and reaching
  • Frequent communication (talking/hearing) and computer use
  • Occasional light lifting (e.g., files)
