Essential Functions: The Geriatric Nurse Coordinator is responsible to maintain the Nurses Improving Care for Healthsystem Elders (NICHE) program in Jersey City Medical Center by using the Age Friendly Health System's 4Ms Framework. This framework focuses on What Matters to the patient and their family, Medications, Mentation, and Mobility. The Geriatric Nurse Coordinator will establish evidence-based standards of care in collaboration with an interdisciplinary team. The Geriatric Nurse Coordinator will also work in partnership with the interdisciplinary staff to optimize the older adult's plan of care throughout the continuum of care and provide evidence base geriatric education to staff, patients, and their family through Geriatric Nurse Referrals. The Geriatric Nurse Coordinator is responsible for guiding and serving as a mentor to other Geriatric Resource Nurses (GRNs) and Geriatric Patient Care Associates (GPCAs). The Geriatric Nurse Coordinator is the disseminator of information about NICHE within his / her organization and is the primary contact for the NICHE national office.
