SUMMARY:

Collaborates with primary care providers and interdisciplinary care team to provide accessible, innovative, and patient-centered care to high-risk patients with diabetes. Primarily through telephonic followup, empowers patients to manage their diabetes by providing education, leveraging technology, and tailoring support to individual needs.

RESPONSIBILITIES:

  • Conducts patient outreach to assess needs, ensure completion of recommended diabetesrelated screenings, establish a selfmanagement care plan, and provide continued support.

  • Utilizes digital tools and emerging technology to facilitate patient selfmonitoring and support remote glucose monitoring for the care team.

  • Coordinates and conducts inperson or telehealth visits for continuous glucose monitoring (CGM) education, data downloads, and other patient needs.

  • Evaluates the healthcare, educational, and psychosocial needs of patients, families, and caregivers, connecting them with appropriate resources.

  • Provides diabetes education to patients newly diagnosed with diabetes.

  • Assists with care coordination related to diabetes management during and after care transitions.

  • Monitors patients' diabetes management plans and collaborates with primary care physicians to optimize treatment.

  • Contributes to ongoing program development while promoting and adhering to ADA Standards of Medical Care in Diabetes.

  • Builds and maintains collaborative relationships with external healthcare providers and facilities.

  • Strengthens coordination and communication with other clinical programs and healthcare partners.

  • Ensures compliance with federal and state confidentiality laws, including HIPAA, to safeguard patient privacy.

  • Adheres to Brown Health Medical Group Primary Care policies and guidelines related to protecting patient demographic, clinical, and financial information.

  • Performs additional duties as assigned.

MINIMUM QUALIFICATIONS:

  • Licensure as Registered Nurse in the State of Rhode Island by the Rhode Island Board of Nursing or licensure as a Registered Nurse in accordance with the Nurse Licensure Compact agreement of the National Council of State Boards of Nursing.

Education and Experience

  • Graduate of an accredited school of nursing with a current Rhode Island RN license in good standing

  • Minimum three years of experience in direct patient care, including experience in diabetes management

  • CDCES or CDOE certification or willingness to obtain within 1 year of hire

  • An equivalent combination of education and experience may be considered.

Knowledge, Skills and Ability

  • Strong interpersonal skills and the ability to collaborate effectively to achieve shared goals.

  • Excellent organizational and prioritization abilities, with strong multitasking and projectmanagement skills.

  • Working knowledge of medical terminology and understanding of medical insurance and benefits.

  • Advanced proficiency with electronic health records (EPIC), Microsoft Office applications (Word, Excel, Outlook), and webbased platforms.

  • Demonstrated critical thinking, problemsolving abilities, and sound clinical judgment.

  • Ability to thrive in a fastpaced environment and adapt to changing needs.

  • Ability to actively participate in quality assurance and continuous improvement initiatives.

  • Ability to work both independently and as part of a team, with strong selfdirection.

  • Strong work ethic and commitment to supporting the wellbeing of patients and families.

  • Ability to communicate with compassion while maintaining professionalism and composure.

  • Competence in performing standard clinical procedures in accordance with established protocols.

Physical Effort and Dexterity

  • This role works primarily in a remote capacity and has daily communications through virtual meetings, some time in the office may be required for meetings, trainings, patient visits etc.

  • Sit and stand for prolonged periods of time

  • Excellent hand/eye coordination

  • Lift up to 15 pounds

  • Manual dexterity to operate a personal computer or laptop as well as standard office machines and equipment as it pertains to one's position

  • State-wide travel and operation of motor vehicle may be required for offsite meetings and trainings

    • Must have a valid driver's license and clear driving record

    • Use of personal vehicle and proof of auto insurance is necessary

Visual Acuity, Hearing and Speaking

  • Capability to transfer information from original source to paper, computer, telephone and in person

Pay Range:

$74,256.00-$148,512.00
