JOB PURPOSE:

The Case Manager will provide comprehensive case management to individuals and/or families experiencing homelessness and/or housing instability by utilizing a traumainformed care and a strengths-based approach with the goal of increasing their quality of life and maintaining the highest possible ability to function within the community.

Qualifications

ESSENTIAL JOB RESPONSIBILITIES:




  • Develop effective, trusting relationships with program participants by utilizing a trauma informed care and strength-based approach. Model positive interactions, verbal guidance, and effective communication.



  • Utilizing the Housing First Model to advocate for and facilitate participant's access to community resources; housing resources, school programs, vocational opportunities or services, recreational activities and relevant mental health services and benefits; assist participants to develop natural resources and contact social support networks. Assist program participants with support for benefits.



  • Develop and maintain comprehensive outreach/working relationship with landlords, property management teams, and community partners on the participant's behalf to develop workable plans to obtain and/or maintain housing.



  • Evaluate program participant's individual and family needs. Complete service plan with the client including housing and other related needs. Coordinate and monitor services, including comprehensive tracking of client activities in relation to the service plan and Housing Inspections.



  • Document detailed case notes in HMIS and Salesforce, daily; maintain comprehensive client files. Prepare reports as requested by Program Director, Corporate Compliance and/or Quality Assurance.



  • Maintain on-going communication with multi-disciplinary teams across multiple programs to assist participants in obtaining and maintaining permanent supportive housing, services to meet needs, and promote stability.



  • Provide needed life skills training on a one-to-one or group basis or connection with partnerships. Assist program participants in developing a budget and monitor payments of necessary bills. Assist with problem solving, social skills, parenting skills, and behavioral modification to meet participant's goals of maintaining housing.



  • Evaluate participants' individual needs. Complete timely service plans with participants including short- and long-term goals; provide follow-up and evaluation of goals. Coordinate and monitor services, including comprehensive tracking of participant's activities in relation to service plans.



  • Meet regularly with supervisor to staff case load. Provide on-going program evaluation and recommendations to the Program Director for continuous growth and quality.

  • Be active in and network at monthly community groups, events, and meetings. Provide presentations at various community groups, events, and meetings within areas of responsibility as needed.



  • Other duties as assigned.

ESSENTIAL QUALIFICATIONS:

EDUCATION:

Bachelor's Degree in Social Work, Sociology, Psychology or related field, preferred but not required.

EXPERIENCE:

1+ years case management experience; 3+ years preferred.

2+ years in a customer service focused environment.

Experience with mental health and/or victims of domestic violence population is a plus.

ATTENDANCE:

Must maintain regular and acceptable attendance at such level as is determined in the employer's sole discretion.

LICENSES:

Driver's License with clear record.

VEHICLE:

Must have daily use of a vehicle without prior notice.

OTHER:

Must be available and willing to travel to various locations and with such frequency as the business need dictates.
