Job Summary:

The Team Lead, Community Health Services, supervises non-licensed staff, including Care Guides, Navigators and Community Health Workers, to enhance community outreach and engagement. This role collaborates with the clinical team to ensure member needs are met and provides team members with the resources and training necessary to assess needs and connect members to appropriate community resources.

Essential Functions:

  • Functions as a "hands-on" supervisor, assisting in day-to-day operations and supervision of non-licensed staff with adherence to departmental expectations and the unique needs of the team.
  • Prepares and utilizes reports to monitor and audit all systems, procedures and the quality of customer services provided, which include, completion of the applicable Health Risk Assessments, other assessment tools (such as PHQ9, CAGE AID, Maternity Screener, etc.) and referral to the appropriate Care Management Team
  • Ensures that non-licensed staff have the most current Care Models, health education, and wellness materials to assess and coordinate services for members
  • Conducts regularly scheduled coaching conversations, providing constructive feedback and strategies for improved performance and member engagement.
  • Collaborate with Manager and Director on the development and implementation of training programs specifically tailored for non-licensed roles, focusing on best practices in community health outreach, engagement strategies, and motivational interviewing techniques.
  • Collaborate with the licensed clinical team to ensure seamless communication and coordination of care for members.
  • Monitors team performance metrics and outcomes, using data to identify areas for improvement and implement corrective actions as necessary.
  • Ensures staffing policies, procedures and SOPs are standardized and are current to meet business needs for non-licensed staff
  • Responsible for recruiting, hiring, onboarding, training, and ongoing support of non-licensed staff ensuring they are equipped to meeting community health goals
  • Supports the Strategic Plan, serves as the technical and content support to align programs/processes (includes supporting UAT activities, autodialer, text/email campaign management) specifically for community health initiatives
  • Supports the development and implementation of community health initiatives that address social determinants of health to promote health equity
  • Assists with implementation of interventions to maintain and/or improve the quality and efficiency of the services provided by non-licensed staff to members
  • Monitors for potential risk, escalate to senior leadership as appropriate, and develop corrective action plans as necessary
  • Perform any other job related duties as requested.

Education and Experience:

  • High School or GED required
  • Managed healthcare experience preferred
  • Experience in a clinical environment preferred

  • Advanced knowledge of Microsoft Word, Excel, PowerPoint, and Visio
  • Exceptional communication skills (both written and verbal) with the ability to present information in a variety of different formats

Licensure and Certification:

  • Must have valid driver's license, vehicle and verifiable insurance. Employment in this position is conditional pending successful clearance of a driver's license record check. If the driver's license record results are unacceptable, the offer will be withdrawn or, if employee has started employment in position, employment in this position will be terminated
  • To help protect our employees, members, and the communities we serve from acquiring communicable diseases, Influenza vaccination is a requirement of this position. CareSource requires annual proof of Influenza vaccination for designated positions during Influenza season (October 1 - March 31) as a condition of continued employment. Employees hired during Influenza season will have thirty (30) days from their hire date to complete the required vaccination and have record of immunization verified.

Working Conditions:

  • This is a mobile position, meaning that regular travel to different work locations, including homes, offices or other public settings, is essential.
    Will be exposed to weather conditions typical of the location and may be required to stand and/or sit for long periods of time.
    Must reside in the same territory they are assigned to work in; exceptions may be considered, due to business need
    May be required to travel greater than 50% of time to perform work duties.
    Required to use general office equipment, such as a telephone, photocopier, fax machine, and computer
    Flexible hours, including possible evenings and/or weekends as needed to serve the needs of our members
  • Over 50% (Mobile) Routine travel required

Compensation Range:
$72,200.00 - $115,500.00 CareSource takes into consideration a combination of a candidate's education, training, and experience as well as the position's scope and complexity, the discretion and latitude required for the role, and other external and internal data when establishing a salary level. In addition to base compensation, you may qualify for a bonus tied to company and individual performance.
