Job Description:

The clinical pharmacist - will provide targeted medication and chronic disease management services, address polypharmacy and deprescribing, improve medication adherence, and support transitions of care. They will collaborate with care teams to improve clinical outcomes, preventative care, and reduce preventable hospitalizations. Responsible for working within a multi-disciplinary clinical team to promote effective, safe, and cost-conscious drug therapy to members.

You Will

Work within a multi-disciplinary team to provide collaborative chronic disease management through direct patient care or through consultation with the primary care team (PCP and/or NP) Diseasestates include but are not limited to Diabetes, Heart failure, Hypertension, Asthma, COPD,Dementia

Comprehensive Medication Management (CMM) Review and optimize complex medicationregimens Identify drug therapy problems and recommend alternatives Monitor for efficacy, safety,adherence, and cost-effectiveness Promote utilization of CPT II & Procedural codes Completecomprehensive medication reviews for patients with specific high-risk medication concerns high riskdisease states, members who are at high risk for rehospitalization, or during transitions of careincluding post hospital discharge Responsible for ensuring compliance with regard to operationsand enhanced member/provider experience This includes review for clinical appropriateness, drugdrug interactions, adverse drug reactions, clinical efficacy, and response, reviewing correspondinglabs, etc Make recommendations to the primary care team (PCP and/or NP).

Transitions of Care (TOC) Perform medication reconciliation post-discharge as required Identify andresolve medication discrepancies Coordinate follow-up care and prevent high risk readmissions(COPD, HF, Falls)

Chronic Disease Management Support management of diabetes, hypertension, heart failure,COPD, etc. Titrate medications per protocol under collaborative practice agreements Provideeducation and adherence counseling to optimize medication adherence (e.g. 90-100 day supply,medication synchronization)

Quality Improvement Prioritize closure of care gaps within chronic disease, medication, orpreventative care measures Contribute both clinically and operationally to achieve Part C & Part D 5STAR rating performance Ensure coding accuracy and capture of Transitional Care Management(TRC), CPT II codes, and/or Snowmed codes within scope of pharmacy related programs/work

Execute protocols regarding appropriate medication use for members, including de-prescribing (e.g.Poly-ACH, COB STAR measures) protocols and high-risk medication review to addresspolypharmacy and clinical appropriateness of medications

Execute pharmacy specific initiatives across the entire membership to achieve quality outcomesResponsible for working with members, clinical care team and SCAN pharmacy team to achieve 5 Star performance on all pharmacy related star measures- medication adherence, SUPD, SPC,TRC, Medication Review, MTM, Poly/ACH and COB

Collaboration & Training: Serve as a resource to providers and staff on pharmacotherapy bestpractices Train staff in safe medication practices and regulatory compliance Provide training to theclinical staff in association with pharmacy specific initiatives and general clinical training at a regularcadence Communicate and work collaboratively with the clinical team to ensure interventions arealigned with pharmacy based goals

Member & Provider Experience: Responsible for ensuring compliance with regard to operations andenhanced member/provider experience.

Maintain professional and technical knowledge by attending educational workshops; reviewingprofessional publications; establishing personal networks; participating in professional societies.

Your Qualifications

Graduate or Advanced Degree required

Doctor of Pharmacy Degree required (PharmD)

Minimum of two years of strong clinical pharmacy experience (required)

Responsible for having a strong clinical background and knowledge of chronic disease states such asdiabetes, heart failure, hypertension, Asthma and COPD (required)

CA Pharmacy Licensure or willingness to apply for California pharmacy license.

Basic Life Support Certification

Vaccine Training Certification Board Certification- Pharmacotherapy, Ambulatory Care, Geriatrics Advanced Practice Pharmacist (APP)

Completion of a PGY1 and/or PGY2 residency or equivalent clinical practice experience.

Technical expertise - Strong analytical skills

Problem Solving - Basic problem-solving skills

Communication - Good communication and interpersonal skills

Proficient with MS Office (Access or SQL, Word, Excel, PowerPoint) and e-mail.

Demonstrates critical thinking and problem-solving skills.

Ability to work independently and in a team environment.

Ability to work under tight deadlines & manage multiple responsibilities independently.

Strong interpersonal skills, including excellent written and communication skills.
