Inova Fairfax Hospital is looking for a dedicated Registered Nurse (RN) Patient Care Navigator - Outpatient Heart Failure to join the team. This role will be full-time day shift: Monday-Friday 8:00 a.m. - 4:30 p.m. (must be flexible). Travel may be required - rotating to our Oakville & Mark Center locations in Alexandria, in addition to Woodbridge and Loudoun. Sign-on bonus and relocation assistance eligible.

Registered Nurse (RN) Patient Care Navigator - Outpatient Heart Failure Job Responsibilities:

  • Coordinates a patient's plan of care which may include appointment scheduling, confirming patient's attendance, transportation, assisting with referral process, and answering patient questions.
  • Acts as a liaison, advocate and point of contact to patients in need of services such as screening, diagnostic treatment, clinical research participation, support services, and follow-up/aftercare plan by helping them access the best healthcare outcomes from doctors, hospitals and others who provide their required services.
  • Refers cases to the appropriate resources for assistance. Anticipates patient's needs and advocates for patient rights and appropriate utilization of services.
  • Conducts comprehensive assessment of the patient's health and psychosocial needs and develops a case management plan in collaboration with the patient and their support system.
  • Educates patient, family or caregiver(s), and members of the health care delivery team regarding treatment options, community resources, insurance benefits, self-care management, etc. to facilitate timely and informed decision-making throughout all phases of the continuum process.
  • Collaborates with multidisciplinary teams to address patient questions and responses about the care process and treatment options.
  • Develops/maintains reliable systems to document, track and monitor patient data.
  • Performs other duties as assigned.

Minimum Qualifications:

  • Education: Bachelors Degree in Nursing
  • Experience: 3 years of nursing experience
  • Certification: Licensed in the Commonwealth of Virginia as a Registered Nurse; Basic Life Support from the American Heart Association

Preferred Qualifications:

  • Experience: Telemetry, ICU, CVICU, IMC - step down; heart failure experience is a plus
